Three staff records were reviewed and found one staff record was missing a physical, evidence of immunity to MMR, and an orientation checklist. One staff record was missing a physical, evidence of MMR, annual PQ registration, evidence of certification in First Aid and CPR, EEC Essentials, and Orientation Checklist.
A review of the evacuation drill logs was conducted and found a drill was not documented in February 2024.
A review of all IHCPs was conducted and found: Two IHCPs were not complete. They were both missing the names of the educators trained to administer the medication(s).
This investigation inspection recorded no violations or advisories.
Four child records were reviewed and found: -One child's record did not have a consent for an (REDACTED). -One child's record did not have a current consent for the (REDACTED) and was missing a consent for the (REDACTED).
The following was observed to be accessible to children: -In INFANT 2 "Yearlings", tubes of diaper cream were stored under the unlocked changing table. All tubes had a label that state, "Keep out of reach of children"; and the kitchen and white noise machine sitting on top of the kitchen were not secure and tippable. -In INFANT 1, two shelves were not secure and tippable.

Medication
Two staff records were reviewed and the following was observed: -One staff record was missing a current physical. -One staff record was missing a current physical, evidence of MMR immunity, and the orientation checklist was not signed and dated by the staff member.
The Staff Schedule was posted in the staff office. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.
The following was observed to be accessible to children: -Rust and paint starting to chip on the gate of the infant area. -The freestanding basketball hoop is not secure and tippable (even with sand or water in the bottom) -The tape is coming unraveled off the poles on the awning in the Preschool play area.
Emergency postings hung in the classrooms were not complete. They were missing address & telephone number of program and name/telephone number for emergency back-up person.
A list of children's allergies was posted in each classroom but they were not complete (missing medication). In a manner that protects the privacy of each child: 1. a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong; and 2. a list of allergies and/or other emergency medical information provided by the parent for each child.
A review of the emergency drill log was conducted and found: A drill was not conducted in February 2024.

A review of educators files indicated the following: -Two educators did not have documentation of completing the EEC Essentials trainings.
The licensee did not have current Building and Fire Inspections from the local building and fire department.

A review of four children's records indicated the following: -One did not have a current consent form to administer medication. -One did not have documented on an IHCP who trained the educator to administer the medication. -Two did not have documentation of which educators were trained to administer the medication to the child. -One did not have documentation of possible side effects of the medication.
The Yearling, Toddler 2 and Pre-K classrooms did not have evacuation procedures posted at each exit. Diagram of escape routes posted in rooms without direct access to outdoor.
This investigation inspection recorded no violations or advisories.
The licensee did not have documentation of the effectiveness of the evacuation drills. All evacuation drills must have the following documentation: date, time, number of children, educators, exit used, and effectiveness of the drill.
The Medication Policy for the program documents that medication should be easily accessible for administration. The medication for one child was inside in the classroom when the child was outside on the playground.

A review of three Individual Health Care Plans indicated the following: -Two did not have documentation of which educators were trained to administer the medication -Two did not have documentation of who trained the educators to administer the medication -One did not have documentation of potential side effects of medication The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment.
Indoor Space
The months of October and November 2019 did not have documentation of the length of time of the staff meetings. In programs with four or more staff the licensee must develop and follow a written plan and must document regular staff meetings of at least two hours per month to consult with educators on program issues; program planning; policies and procedures; parent communication; implementation of 606 CMR 7.00; children’s behaviors and meeting the individual needs of children.

Food Storage, Sanitation & Safety
A review of four staff records indicated the following: -Two did not have evidence of registering annually with the Department of Early Education and Care. -One did not have evidence of completing the required Strong Start Training's. -Two did not have evidence of completing orientation to Early Education and Care approved by the Department. -One did not have evidence of being immunized in accordance with the recommendations of the Department of Public Health.
A review of the incident/injury log indicated the following: -Six injury reports were not documented in the incident/injury log. -Three injury reports were documented in the incident/injury log however, there was not evidence of the reports in the children's records. -One injury report did not have documentation of the child's date of birth and the date of the injury.
A review of nine children’s records were reviewed and indicated the following: -Three did not have evidence of annual physical examinations.
The following was observed and accessible to children: PRE-K CLASSROOM: -Shelf holding the large bucket of legos was not secured. -Small wooden shelf holding manipulatives was not secured. -The couch has a rip in the material approximately three inches in length` TODDLER 2 CLASSROOM: -Plastic kitchen was not secured. INFANT CLASSROOM: -The infant / toddler table with bucket seats was missing one safety belt.
-In the Yearling Classroom on the window sill, there were numerous dead bugs.
A review of medication stored in the Infant Classroom and a discussion with staff indicated that one unlabeled container of Acetaminophen did not belong to any child enrolled at the program.
The Evacuation drill logs did not include documentation of the effectiveness of the drills. The educator must hold practice evacuation drills with all groups of children monthly. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
An open package of graham crackers were in a cabinet in the pre-k classroom.
