The following was observed to be accessible to children during the visit: In INFANT B, the paint/plaster on the windowsill is chipping off. The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning.
The Preschool 1B attendance for 2/12/2026 was reviewed and found one educator was alone and with up to 14 children during the times of 8:31 and 8:47am. For a fixed age group of Preschoolers aged 33 months to school age (Full Day), the ratio is 1 teacher to every 10 children.

Supervision of Children
On 11/25/2025, an educator did not exercise the appropriate supervision of a child in their care in order to ensure their health and safety at all times. The educator accidentally gave an infant child the wrong bottle. All licensees and educators must exercise appropriate supervision of the children in their care in order to ensure their health and safety at all times. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. This was previously cited during a visit on 9/11/2025 (visit 168421).
During the visit, the program stated they offer parent teach conferences but as on 12/2/2025, they were unable to provide evidence that conferences are offered. Parent conferences was part of the concern in the reported complaint on 11/21/2025 in case 00167154.

The following was observed during the visit: In PRESCHOOL 2B, three children's water bottles were not labeled with their names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
One child's (REDACTED) was not readily available in the classroom. It was stored in the director's office at the time of the visit. The licensee must ensure that the following are easily and readily available at all times, and accompany the children anytime they leave the facility in the care of staff: emergency or life-saving medications, such as asthma inhalers and epinephrine auto-injectors, for any children for whom they have been prescribed.
The following was observed during the visit to be accessible to children: In the TWADDLER A bathroom (unlocked and open ajar), an educator's pocketbook was sitting on a chair.
One child's record is missing an IHCP, doctors (REDACTED) action plan, and medication consent form. The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered. The written parental consent and the licensed health care practitioner authorization shall be valid for one year, unless withdrawn sooner. Such consent and authorization must be renewed annually for administration of medication and/or treatment to continue.

The following was observed to be accessible to children during the visit: In PRESCHOOL 2A the purple wall under the window had chipping paint and plaster. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.

The parents had provided one (REDACTED) but the doctor's allergy action plan states to (REDACTED) if symptoms continue after five minutes. emergency medications such as epinephrine auto-injectors must be immediately available for use as needed.
A hole was observed in the wall above the sensory table and within reach of children in PRESCHOOL 1B and plaster was visible.
During a visit conducted on 4/22/2025 (#163415), Technical Assistance was provided: A child was in attendance in the PRESCHOOL 2B classroom, but this child was not on the enrollment lists the program previously provided to EEC. In the future, if any child not listed on the pre-approved lists is identified to be in attendance at the program, the program will be referred to the EEC Regional Director for review. During a visit conducted on 6/10/2025 (#161227): A child in attendance in the PRESCHOOL 3 classroom, but this child was not on the enrollment lists the program previously provided EEC. This child was not found on the pre-approved lists. Corrective Action Plan Submitted: No new students will be in attendance who are not on the pre-approved list that was provided to EEC. The Learning Experience in Foxboro will only have children in attendance who are on the pre-approved list that EEC has a copy of. The Executive Director, Director and Assistant Director fully understand that no new children are to be enrolled at this time and only children who are on the pre-approved EEC list can attend The Foxboro Learning Experience. The Executive Director will verify this with her weekly enrollment report. Today's visit conducted on 7/11/2025: A child in attendance in the INFANT A classroom, but this child was not on the enrollment lists the program previously provided EEC. This child was not found on the pre-approved lists.
During the previous visit conducted on 6/10/2025 (#161227): Three staff records were reviewed and found one staff record did not have a current physical (last dated 5/2023) and did not have evidence of immunity to MMR. Corrective Action Plan submitted: The Center Director has completed an assessment and update to all staff files. The 3 staff files are completed now with up-to-date physicals and MMRs. The Center Director will carefully review the staff files monthly. She will utilize the checklist to ensure files are complete and in good standing with applicable expiration dates. This system will be verified by the Executive Director monthly. Expiration dates for staff documents will be entered into The Learning Experience’s CORE system to alert the directors and alert the employee when a document is about to expire. Today's visit conducted on 7/11/2025: Two staff records were reviewed and found one staff record is missing evidence of one MMR vaccine (two are required).

During the previous visit conducted on 6/10/2025 (# 161227), The names of staff on the current Staff Schedule (uploaded by program on 6/13/2025) were cross referenced with the names in EEC's BRC Navigator. One educator is working at the program and has an expired BRC. It expired on 6/8/2025. In addition to the information required by 606 CMR 7.04(5), the licensee must maintain in each personnel record: evidence of completed background record checks, as required by 102 CMR 1.05(2) and 606 CMR 14.00. Corrective Action Plan submitted: The teacher has completed her fingerprinting appointment, and the administrative team is waiting for her letter of suitability for her return to work. The Assistant Director will be checking staff files monthly to ensure all information is up to date and using the staff records checklist. We will also be putting this information into the CORE system at TLE Foxboro so we can be reminded of any BRCs expiration dates in the future. During today's visit 7/11/2025: The educator's names listed on the posted Staff Schedule during the visit were cross referenced with the names in EEC's BRC Navigator. One educator listed on the Staff Schedule and written the attendance sheet for PRESCHOOL 1B does not have an EEC BRC.
During the previous visit conducted on 5/23/2025 (#163417), Technical Assistance was provided: In INFANT A it was observed that the cribs were set up too close together. Educators must ensure that: there is a distance of at least two feet between each crib or cot, or there is a distance of at least three feet between children’s faces while resting or napping. During today's visit 6/10/2025: the cribs remained in the same set up.
The following was observed to be accessible to children: In TODDLER A, the purple wall under the window has a long crack in the paint and plaster; in PRESCHOOL 2, the white bookshelf in the middle of the room had a large spot of tape residue that was sticky; and in PRESCHOOL 3, the student world globe had a large piece of the surface ripped away.
Five child records were reviewed and found one child’s record did not have a current enrollment form, CPR/First Aid consent, developmental history, and transportation plan (expired 6/2/2025) and their IHCP was incomplete (missing side effects, who was trained to administer the medication and who they were trained by). Two child records did not have physicals. One child’s Dr. Allergy Action Plan was not current (expired 4/23/2025). Another child’s IHCP was incomplete (missing the names of educators trained to administer medication and who they were trained by). In addition, three additional children’s chronic medical condition paperwork was reviewed and found two IHCPs were not complete as one was missing the names of the educators trained to administer the medication and who they were trained by and the second was missing the date the parent signed the form and one child’s Dr Allergy Action Plan was not current (expired 5/2025), the IHCP was not complete (missing name of educators trained to administer and who they were trained by).

One child's emergency medication to treat seizures was not readily available in the child's classroom. It was in the director's office. In addition, three other children's antihistamines were not available but were listed on their allergy action plans. Notwithstanding the provisions of 606 CMR 7.11(2)(e), emergency medications such as epinephrine auto-injectors must be immediately available for use as needed.
An educator in TODDLER A changed a child's diaper and did not immediate wash the child's hands. The child was placed onto the ground to walk around the classroom where the child proceeded to touch a bookshelf. The licensee must ensure that educators and children wash their hands with liquid soap and running water, using friction, in accordance with DPH guidelines. Hands must be dried with individual or disposable towels or automatic hand blow-dryers. The use of common towels is prohibited. Educators and children must wash their hands at least at the following times: after toileting and diapering.
One child's medication consent forms was not completed (missing side effects and storage instructions).
During the visit, classroom attendance sheets reflected two different children were not in consistent classrooms. One toddler child was signed into three classrooms (Twaddler A, 7:49am-8:02am, Twaddler B, 8:02am-8:31am, and Toddler B, 8:31am- (present during visit). One preschool age child was signed into two classrooms, twice per classroom (Twaddler A, 7:58am-8:10am, Preppers, 8:10am-8:59am, Back to Twaddler A, 9:01am-9:10am and Back to Preppers, 9:12am- (present during visit). Each child must be assigned to a consistent group with consistent staffing.

The following was observed during the visit: In PRESCHOOL 1B, a child entered the bathroom, shut the door completely and shut off the light. The Curriculum Coordinator stated that this child repeatedly exhibits this behavior which is a safety concern. In addition to interacting with children as required by 606 CMR 7.05, all licensees and educators must exercise appropriate supervision of the children in their care in order to ensure their health and safety at all times. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities.

The following was observed to be accessible to children: In PRESCHOOL 1A, a stapler remover was found in an unlocked drawer; in PRESCHOOL 2A, two sleep mats had rips, four cabinet doors are peeling at the bottom and two of these doors have broken hinges; in PRSCHOOL 1B, two sleep mats had rips; and in PREPPERS, four floor tiles in front of the exterior door were broken. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.
The following was observed to be accessible to children: In PREPPERS, the top of the green wall below the window had exposed plaster and chipping paint. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.
The parental consent for a child's antihistamine was not available during this visit. Written parental consent is required for all prescribed and oral non-prescription medication.

The following was observed in the PS1B classroom: Two children were in the bathroom with the door open but neither of the two educators were supervising them or could see them from where they were located. One child was using the toilet and the other child sat in the chair inside the bathroom for a minute and then stood up. One educator was trying to gather the children on the rug for a book or song and the other educator was cleaning up the toys left of the tables. In addition to interacting with children as required by 606 CMR 7.05, all licensees and educators must exercise appropriate supervision of the children in their care in order to ensure their health and safety at all times. Such supervision must include, but not be limited to, indoor and outdoor activities, mealtimes, naptime, transportation, field trips, and transitions between activities. Educators must be positioned to maximize their ability to see and/or hear children in their care. When toilet facilities are used exclusively by the licensee's program and are not available to the public, an individual child of the appropriate age may, with permission from the educator, go to the bathroom independently. The educator may only allow one child at a time to use the bathroom independently.

The following was observed: Licensor observed an infant child in Infant B get handed over the half wall to an educator in Infant A. The one educator is Infant B accepted this fourth infant (from the parent) and once the parent left the classroom, the educator handed the child to the educator in Infant A. The child was never signed into Infant B but the educator was alone with four infants until the parent left. Infants (up to 15 months old) (Full or Half Day) Ratio is 1: 3, one additional educator for four seven infants. Ratios must be maintained at all times.

INFANT A stated they have not gone outside since before the previous EEC visit on 3/28/2025 despite the warmer weather in the last couple of weeks. As appropriate, children must participate in the development of the plan, and the plan must provide for: daily indoor and outdoor time periods, weather permitting, which include both small and large muscle activities.
The following was observed in TODDLER A: a child's water bottle was observed to be laying in the classroom sink. Because the classroom only has one sink and this sink is used for handwashing after diaper changing, it cannot be used for anything else. Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service.
The following was observed to be accessible to children: In PS 2A, a plastic toy bin was broken and had sharp edges; In PS 2B, dust and food crumbs were visible on the white shelves throughout the classroom; and in TODDLER B, one drawer near the changing table was unlocked and contained loose plastic bags and a roll of trash liners.

The attendance in Make Believe Blvd PRESCHOOL 2B was inaccurate at the time of the visit. Sixteen children were marked present but fifteen children were in attendance when licensor arrived. This was corrected during the visit.

The following was observed to be accessible to children: In TODDLER B, the unlocked cabinet under the sink area contained a staff purse, three cleaning products with the labels "Keep out of reach of children", and a bag of the children's sunscreen, in TWADDLER A, a staff purse sat on the floor to the left of the sink; in PRESCHOOL 1B, a bottle of bleach/water was sitting on the half wall next to the change table and a pair of teacher's scissors was in an unlocked drawer next to the sink; in the PRESCHOOL 2A bathroom, the floor laminate behind the toilet was broken and in the classroom an open box of gloves as well as a plastic bins contained loose gloves sat on the right side of the sink.
Three rest mats in PRESCHOOL 2A and five rest mats in TWADDLER A were observed to contain rips. The licensee must keep all equipment, materials, furnishings, toys, and games clean and in a safe, secure, and workable condition.

Observations from original visit on 2024-06-13 00:00:00: None of the classroom first aid kits had all of the required items. Corrective Plan received on 6/25/2024: First aid checklist were handed out to all staff so they could understand what needs to be put into their emergency medication bags. Once filled out center management went through and filled each classroom bags with the proper supplies needs from the checklist. Visit on 7/16/2024: The First Aid Kits in INFANT B and PS 2B were missing scissors.
Observations from original visit on 2024-06-13 00:00:00: The following was observed to be accessible to children: -In PRESCHOOL 1A, dust and crumbs were observed on the surface of the wooden shelves and in the corners of the floor. -In PRESCHOOL 2, a pair of adult scissors was sitting on a windowsill, an unlocked drawer contained three loose plastic bags, and three open boxes of gloves were found (2 boxes on a low shelf and one box on the windowsill closest to the bathroom). -In TWADDLER A, two ceiling tiles were stained and the ceiling vents were dusty. Corrective Plan received on 6/25/2024: All teachers reviewed classroom cleaning checklist which will be signed off by them and assistant director twice a day and at the end of each week when passed in- to ensure all classrooms are neat and clean with no safety hazards present. Work orders went in for ceiling tiles and broken cabinet doors, which Maintenace personal fixed on 6/15/24. While Licensor was conducting visit- Director removed the adult scissors and box of gloves out of reach of children. Director and assistant director addressed these items to staff the day of the visit verbally. Center Director again verbally went over this noncompliance during a staff meeting conducted 6/24/2024. Visit on 7/16/2024: In TWADDLER A, the ceiling vent still had visible dust.

Observations from original visit on 2024-06-13 00:00:00: Upon review of child records, it was found that three month progress reports are not being conducted on infants as required. Corrective Plan received on 6/25/2024: Center Management (curriculum coordinator) Developed an additional progress monitoring report that will be done in the summer months (June-August) to ensure that all infants progress is being monitored every three months. Visit on 7/16/2024: Progress reports on infants have not been conducted since the last visit. The Assistant Director stated the online platform the program uses for progress reports cannot be used during summer months.
Observations from original visit on 2024-06-13 00:00:00: Three staff records were reviewed and found: -One staff record did not have evidence of two MMR vaccines (they had one), the orientation checklist was missing the number of hours it took to complete, and evidence of professional development and annual evaluations were not provided. -One staff record had an orientation checklist that was missing the number of hours it took to complete, and evidence of professional development and annual evaluations were not provided. -One staff record had an incomplete orientation checklist (items completed were not checked off, signature of admin, and hours to complete). Professional development and annual evaluations were also not provided. Corrective Plan received on 6/25/2024: Center Director will use the EEC staff record checklist to ensure that all staff will have all required paperwork prior to starting and update it as needed. Center Director will complete staff orientation checklist upon hire and monitor and document professional development hours. Yearly evaluations will be documented and kept in the staff's file. Center Director worked with field leader to develop excel sheet which included all requirements for staff files- this excel sheet will turn yellow when a item on staff record checklist is going to expire in three months and turn red when it is going to expire in one month. Director will maintain this document by adding bi-monthly notification to health and safety calendar. Visit on 7/16/2024: -One staff record did not have evidence of two MMR vaccines.
Observations from original visit on 2024-06-13 00:00:00: After observing the TWADDLER B (Tod/PS) attendance and confirming with the teachers, it was discovered that this classroom operated with ten children this morning. Nine children is the maximum group size for this age grouping. After observing the PREPPERS (Tod/PS) attendance and confirming with the teacher, it was discovered that one teacher was alone with seven children this morning. Corrective Plan received on 6/25/2024: Center Management met on 6/14 to discuss the findings of staff/classroom out of ratio in the morning in the Twaddler and prepper classroom. Two staff schedules changed after looking at the children's consistent drop off times in the morning to ensure that there is enough staff for the number of children entering the center/classrooms. Teachers were given clear expectations to when a child comes which would make them out of ratio. (center director went over this policy with each staff individually and at staff meeting on 6/24/24-staff members signed off on policy on 6/24/2024) If a staff member is alone and a parent comes to drop of a child, which would make them out of ratio, they are to kindly tell the parent to see center management to inform them. Center Director communicated this policy with all active families on 6/20/2024 via text message and e-mail. Visit on 7/16/2024: Four infant children are currently enrolled in the INFANT B classroom. No more than three infants can be enrolled in a mixed Infant/Toddler classroom.
Observations from original visit on 2024-06-13 00:00:00: None of the emergency medications had medication consents signed by the parents. Corrective Action Plans received on 6/25/2024: Center Director switched all TLE medication forms for EEC Medication consent forms. All parents of children with IHCP's signed new medication consent forms on 6/14 and 6/17. Visit on 7/16/2024: Consents for the medications were not available during the visit.
Observations from original visit on 2024-06-13 00:00:00: The following was observed to be accessible to children: -In TWADDLER B, the blue wall has paint and plaster chipping. -In PREPPERS, the green wall has paint and plaster chipping. Corrective Plan received on 6/25/2024: Work orders were put into maintenance regarding chipped plaster and paint. The work was completed on 6/15/2024. Staff members were verbally reminded by center director at staff meeting on 6/24/2024 that all safety work orders that are noticed are to be put on their weekly cleaning checklist and handed in to center management. Visit on 7/16/2024: The wall in the PREPPERS classroom was fixed but the wall in the TWADDLER B classroom still had plaster visible and within reach of children.

PRESCHOOL 2 was observed to transition back to the classroom from outdoors and three children were observed to not having taken their water bottles. One of the teachers stated that the child did not want to bring the water outside. Educators must offer water to children at frequent intervals and upon request by children.
The following was observed which indicated the infant classrooms were not consistent: -One child from INFANT A had been switched to INFANT B and one child in INFANT B has been switched to INFANT A yet their names were still posted in the classrooms. Each child must be assigned to a consistent group with consistent staffing
The attendance in INFANT A was incorrect. Seven children were present in the classroom but eight were checked in on the attendance.
The trash bin in the PREPPERS was observed to be without a cover.
None of the classroom first aid kits had all of the required items.
None of the emergency medications had medication consents signed by the parents.
Antihistamines were not available for three children with allergies and antihistamines listed on their Allergy Action Plans.
The following was observed to be accessible to children: -In TWADDLER B, the blue wall has paint and plaster chipping. -In PREPPERS, the green wall has paint and plaster chipping.

All emergency medication was observed and found: -An (REDACTED) for a child was in the original packaging but the pharmacist label had been removed. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed.
A review of four staff records was conducted and found: -Bi-monthly observations were missing from the four staff records. Bi-monthly observations were not conducted consistently from March 2022 to September 2022.

The following was observed to be accessible to children: -In TODDLER A, a white bookshelf had two holes and particle board was visible. -In PRESCHOOL 2A, a hole is visible in the wall next to the door and the plaster is exposed and speaker wires were hanging loosely and longer than six inches from the device to the outlet. -In PRESCHOOL 3, two bookshelves had holes and particle board was visible and the two tall shelving units to the left of the sink were not secure and tippable. -In PRESCHOOL 1B, the white bookshelf had holes and particle board was visible, a pair of adult scissors was laying on the half wall ledge above the changing table, and the baseboard was detaching. -In TWADDLER A, yellow paint is chipping to the left of the outside exit door. -In TWADDLER B, two holes are in the wall to the right of the toy kitchen set. -In TODDLER B, two bookshelves had holes and particle board was visible.
The following was observed to be accessible to children: -Two freestanding basketball hoops were not secure and tippable. -Ten bolts on the fencing and gate between the two outdoor play spaces are longer than three threads.
