There were hazards in the preschool playground, including rust on the edges of the steps and platform on the climber, a pole at the top of the fence is not connected, and nails are protruding on the corners of the wood barrier surrounding an area.
There were hazards in three classrooms including one shelf in each room was not installed and tippable and two cabinets were not locked and toxic substances were accessible to children. In the community room, the wood radiator cover was damaged in the bottom corner and it can easily open and cause an injury to children.

The educators reported that progress reports are completed for children once a year. It is required that progress reports are completed every 6 months for toddlers and preschoolers.
It was observed that some areas on the walls in the interior of the building have chipped paint.

It was observed that cords were accessible to children, furniture was not bolted, and that cabinet locks were missing in some rooms.
The fall zones did not have an adequate depth of impact absorbing material under and around the preschool climbing structure. The depth was 6 inches in two locations. There was also a slide intended for residential use on the preschool playground and it was located in an area with no fall zone.
During a review of children's records, it was found that three children were missing the EEC Individual Health Care Plan and the medication consent form signed by a parent.
The program did not have any records of monthly fire drills.
The fencing between the toddler and preschool playgrounds and one of the playground gates contained head entrapment hazards for children.
