It was observed that hazards were present in the indoor space. It was damp in the cabinet under the sink in the Oak Room and there was an odor and mold present on the washcloths, several safety latches on cabinets were not working properly, cords were accessible to children, and corners of rugs were curled posing a potential tripping hazard.

Children's allergies were not posted in a manner that protects the privacy of each child in the Oak Room.
There were no records that evacuation drills were conducted at the program for May and June.
The program did not have a parent's signature on an Individual Health Care Plan for one child, and it did not have doctors consent for Benadryl for another child with an allergy.
Equipment is clean, safe, secured, and poses no hazard.
It was observed that the climbing structure in the Oak Room is not appropriate for toddlers. The height of the platform exceeds 18 inches, impact absorbing material under and around the climber is inadequate, and the fall zones do not extend 6 feet on all sides.

It was observed that an air conditioner cord longer than 6 inches was accessible to children in the Oak Room next to the climbing structure.
The fall zones near the climbing structure did not have an adequate depth of impact absorbing material in the small outdoor space in the back of the facility. It measured approximately 6 inches. The wood chips were compressed in the large outdoor space in the back of the facilty near the climbing structure.
An allergy for one child was written on a wipe off board in the classroom. It was not posted in a manner that protects the privacy of the child.
A child with allergies in the Forest Room is missing two signed medication consent forms for prescription Benadryl and an Epipen.

It was observed that a 3-month-old child was swaddled with their arms not free to move and another child had a pacifier with a fabric cord while sleeping in a crib.

During an inspection of the playground, the licensor measured the impact absorbing material (wood chips) around the climbing structure in two places, and it measured 7 inches.
