Two medication consent forms were missing parent/guardian signatures.
Children’s records did not have a copy of their IHCP’S. One child with asthma and Allergies was missing their IHCP’S.
The staff schedule form did not have the First Aid certification expiration dates listed. Three Staff Files reviewed were missing evidence of current First Aid.
One child did not have their prescribed Albuterol on site.

Three children’s files were reviewed three of the children have chronic Health conditions, but did not have individual healthcare plans in their files. All documentation in the children’s files reviewed was expired.
Two children with documented chronic health conditions did not have an Individualized healthcare plan.
