The infant/toddler classroom had eight children, there were seven children signed into the attendance.
This investigation inspection recorded no violations or advisories.

Medication
The Albuterol observed was not labeled and was not in the original container with the prescription label. The educator reported she did not record medication administration. The children did not have a Medication Consent form for the insect repellent offered by the program.

The fall zones in the Preschool playground and in the Infant/Toddler playground where not Adequate. The wood chips on on the Infant/Toddler Playground were wood chip nuggets, which is a choking hazard for infants and toddlers.

The staff records checklist was not available for review during the visit. It was reported that due to all the new staff it needs to be updated.
During the visit the Epi-Pen in the toddler room did not have any parental consent for administration of the Epi-pen or Allergy action plan for treatment.
Medication Administration Policy
During the review of children's Individual Health Care Plans (IHCP), it was learned that not all plans had documentation of all the required information, including what educators were trained about the plan and by whom they were trained.

On the day of the visit, there was not a current staff schedule posted in an area easily visible to educators, parents and visitors.
The Preschool playground had several rail road ties that were broken and the screws were protruding out of them. It was reported they are all being replaced this month. The Emergency phone numbers were not posted in the classrooms.
One staff file was missing a physical and another did not have a application, interview notes or references.

On the day of the visit two boxes of opened crackers had not been stored in a safe, sanitary way.
On the day of the visit, there was a current Building Inspection posted. There was not evidence of a current fire inspection.
During the playground inspection it was learned that there was not an adequate amount of impact absorbing materials in the fall zone of the large structure in the preschool classroom or in the fall zones of the equipment in the infant/toddler playground.
