An epinephrine auto-injector and a prescribed inhaler were not in the the containers in which they were originally dispensed and with their original labels affixed.

The children's records were reviewed for the four children present. Two children did not have lead testing, one child needed an updated physical, one child needed their immunization records and one child with a medical condition required an individualized health care plan.

One child enrolled has a medical condition which requires an individual health care plan which the provider did not have on file.
Provider had only completed five hours of ongoing professional development, provider was required to have a minimum of 10 hours.
The refrigerator temperature was 46 degrees.

There were not enough materials for children all of ages.
First aid kit was missing the thermometer. Provider added the thermometer to the kit during the visit. Also, the first aid kit was missing the emergency cards for two children.
Children’s extra clothing were in their own labeled bags. However, they were all piled on the top of a shelf. The educator does not have a place to store personal items separately, as required.
There was a table, where children were eating lunch when licensors arrived, blocking the second emergency exit.
There was an electric cord accessible to children next to the second emergency exit door.
There was no evidence of a well balanced curriculum and routines for children of all ages, during the visit. Children were having lunch when the licensors arrived. After that, they were just watching TV, and did not seem to know what to do next. There were not enough materials accessible to children. Provider showed me a routine schedule. However, it did not seem to match with what was happening during the visit. Provider also showed some activities with glued shapes and drawing that were done by the children, however, there was no evidence of a developmentally appropriate curriculum happening on a consistent basis for all children.
The Educator wasn't aware that she had to practice and document monthly fire drills.
The Licensor observed a (REDACTED) sleeping in an infant swing. When asked, the Educator stated that the child sleeps in a swing at home. The Educator stated that the (REDACTED) won't sleep in a playpen or any other surface. The (REDACTED) will get up and disturb the other children. Restraining children in any equipment is strictly prohibited.
The Educator wasn't aware that the parent has to sign the medication form in order to dispense medication and the Educator wasn't aware that she had to document each dose given.

The baseboard in the kitchen was rusted. A portable heater was observed in the bathroom. Portable heaters are prohibited.

The Licensor observed an exercise bicycle in the kitchen that was accesible. The Licensor observed cords in the kitchen that were loose and accessible. The Licensor observed plastic bags, easy off, raid and other aerosol sprays under the kitchen sink. The Licensor observed plastic bags in one kitchen draw accessible. The Licensor observed clorox wipes, diapering powder, creams and vaseline accessible on the diapering table. The Licensor observed plastic gloves that were accessible near the diapering table. The Licensor observed listerine, medication and a cord accessible in the bathroom.

Progress reports
The Licensor asked the Educator about her daily routine and curriculum. The Licensor observed a small plastic covered bin with very few play materials. The Educator stated that each child is given a bin to play with. All other play materials are stored away due to the pandemic. There were very few play materials accessible for the children to play with and no outdoor play equipment. When asked what the children did the day of the visit, the Educator showed the Licensor one sheet of plain white paper that two children had made circles and drew lines. When asked about her curriculum, the Educator stated that she doesn't plan activities. The Educator had a difficult time explaining her daily routine. The children didn't have access to puzzles, blocks, fine motor materials, etc.
The Educator currently stores the children's personal belongings in a bag in her bedroom floor. The Educator doesn't have a place to store personal items, as required.

The Educator's emergency list is missing her telephone number and her address that are required.
The Educator is not completing progress reports, as required.
The Licensor asked about the children's activities. The Educator stated that the two younger children who are (REDACTED) are kept in playpens for 2 hours at a time because of the pandemic. The Educator stated that her space is small and the (REDACTED) aren't able to stay in a confined space unless they are placed in the playpen.
The Educator is not using two surfaces for diapering as required.
