Supervision of Children
During the visit, there were children sleeping in the T/PS room. The educator was out of the room in the kitchenette, getting coffee. She walked into the T/PS room for a few minutes and then walked back out. The children were unattended during that time. In the I/T room, there were 7 children and 1 educator when I arrived to the classroom. Approximately 1 minute later the second educator, who had been in the kitchenette, returned to the room.

The following was observed: A review of the INFANT/TODDLER attendance record indicated that one child was not signed in; this was immediately corrected by staff. In the PRESCHOOL classroom, there were 10 children present with one Educator. The program was out of ratio. (This was previously cited on 11/28/22).
The program did not have evidence of a current Building and Fire inspection.
It was observed that diapering and toileting procedures were not posted in the INFANT/TODDLER classroom and the CHILDREN'S BATHROOM.
It was observed that in the INFANT/TODDLER classroom, the cabinet underneath the changing table was unlocked. Inside, were plastic bags and an open box of gloves accessible to children. This was immediately corrected by staff.
A review of one individual health care plan (IHCP) indicated that it was not fully completed to indicate which staff are trained and designated to administer the medications in an emergency.
A review of two medications indicated that they were not stored with their original container and prescription label.
There was no evidence of a parental consent form for a child with prescribed medication stored.

During the visit, the program was out of ratio in the PS room. The Director had 12 children alone. She stated that her staff had called out and she was not able to get a substitute staff.
During the visit, a(REDACTED) child was observed sleeping in a crib, with a pink blanket covering her.
In the PS room, the wood slat flooring were separated near the tables. This is a potential trip hazard.
