It was observed that there was a plunger located next to the toilet in ROOMs 1 and 3, and there was a chipped windowsill in ROOM 1. These were all accessible to children.

There was no evidence of a current fire inspection from the local Fire Dept. The program did have a current fire alarm and sprinkler report.
It was observed that there was chipped paint in ROOM 1.
The following was observed and accessible to children: The plastic tool bench was unsecured and tippable, and unknown vegetation was growing through the back fence.
It was observed that evacuation procedures were not posted at the internal door in ROOM 2.

It was observed that a tub of disinfectant cleaning wipes was located on the counter accessible to children.

The following was observed to be accessible to children, in: STARFISH: - A loose plastic bag in a drawer to the right of the sink. -A pair of adult scissors in the top left drawer. -Tape residue on the bookshelf containing bins of toys. -Loose computer cords longer than six inches laying on the rug next to a basket of books. SEAHORSE: - Tape residue on top of two bookshelves.
The following was observed to be accessible to children in the open kitchen: -Electric stove -Coffee machine plugged in and at the edge of the counter -Adult scissors and a screw driver in a drawer closest to the door -Pizza cutter in the drawer to the right of the small sink -Five sharp knives in a drawer to the right of the stove -Dish washing soap, hand sanitizer bottles and wipes in the two cabinets below the two sinks
The following was observed to be accessible to children in the STARFISH: -Four bottles of Bleach and spray cleaner in unlocked cabinet on right side of room.
Evidence of the vehicle's registration and inspection used for transportation has not been provided yet.

Transportation Drivers
The gate used to make the kitchen inaccessible to children had been moved and allowed the kitchen to be open to children.
The Building Inspection has been provided and expires on 11/30/2022. The Fire Inspection has not been provided.
Transportation Vehicles
Three children's records were reviewed. One child was missing the consent for staff to administer first aid, CPR, and transport to a medical facility.
A child in the MANATEES room has documentation that she has (REDACTED) and staff is to check her temperature daily per the doctor. The Assistant Teacher in the classroom and the Nurse were unaware of this medical need. Licensor recommended the program call the doctor or parent to confirm if this should be continued or discontinued and to get it in writing from the doctor.
