Three educators did not register annually with the Department of Early Education and Care.
During a monitoring visit at the program on 12/2/25 two educators were assigned to supervise children in classrooms without having a Suitable Background Record Check. All educators must have a suitable BRC to be assigned to supervise children in a classroom.
Three educators admitted to not having completed the Strong Start EEC 2.0 Essentials Trainings which were due by 6/30/25. Educators should not be in the classroom until they have completed these trainings.
A review of Individual Health Care Plans indicated the following: -One did not have documented who trained the educators to administer medication to the child. -One did not have documented which educators were trained to administer the medication to the child.

A review of the children's records indicated the following: -One did not have documentation of lead paint screening as required. -One did not have documentation of a current physical examination as required. -One did not have a current Individual Health Care Plan as required. -One did not have current enrollment paperwork. The paperwork expired 9/20/24.

A review of four child records was conducted and found: -One child's record was missing evidence of a current physical and immunizations.
A review of the Evacuation Drill was conducted and found the time of day the drills were conducted was not recorded. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
The following was observed in the outdoor space accessible to children: -The fall zones at the bottom of the green slide and long red slide had an insufficient depth of mulch. Approximately one inch of mulch was visible. At least nine inches of mulch must be maintained in the fall zones of the slides. Loose-fill surfacing such as mulch requires frequent maintenance to ensure surfacing levels never drop below the minimum depth. Areas under swings and at slide exits are more susceptible to displacement; special attention must be paid to maintenance in these areas.
The following was found to be accessible to children: -In INFANTS, the brown and tan throw rug in the entrance of the room was frayed at one corner and is a choking hazard to children if the strings tear off.
A review of two staff records was conducted and found: -Two staff records were missing bi-monthly observations between June and November 2023. The last observations were dated 6/15/2023.

Emergency medications readily available
The following was found to be accessible to children: -In the TODDLER CLASSROOM, paint is peeling along the bottom of the wall between the baseboard and the play kitchen on the right side of the French doors.
Currently one child in the program has emergency medication and it was kept in a closet near the kitchen at all times. This location is not considered readily available at all times as the medication is not taken outside and if the medication is needed, an educator would need to leave the classroom to access it. Emergency medication should be kept in the classrooms so they are readily accessible to staff in the event the medication is needed. Medication should also go outside with the group when they transition outdoors.
This renewal - monitoring visit inspection recorded no violations or advisories.
A review of children's records indicated the following: -One did not have current enrollment documentation. Authorization and Consent, Face Sheet, and Developmental History were not complete and more than a year old. -One child did not have evidence of an annual physical examination. -One Individual Health Care Plan did not have the following documentation: -Potential side effects of medication -Potential consequences of not administering the medication -Which educators were trained to administer the medication -Who trained the educators to administer the medication
The licensee did not have an evacuation procedure posted inside the infant crib classroom.

A review of two children's records indicated the following: -Two did not have documentation of current developmental history forms. -Two did not have documentation of current physical examinations.
In the bathroom on the sink was Coastal Breeze Hand Lotion which had documentation on the bottle to "Keep out of the reach of Children".
A review of hazardous items in the preschool classroom outside the emergency door was a dog run lying on the ground.
-One Individual Health Care Plan (IHCP) was incomplete. The child's name and parent signature were the only documentation on the form. Additionally, the same child has had three prescription medications administered to him without without current documentation on an IHCP's. The licensee must maintain as part of a child’s record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child’s health if the treatment is not administered.
