A review of emergency evacuations indicated the licensee did not perform an emergency evacuation in the month of February 2025.
A review of children's records indicated the following: -Three did not have current and completed medication consent forms for their emergency medications. All children with medications must have a current and completed medication consent form to administer medication in an emergency. -One did not have documentation of a current physical examination from a certified physician. -One did not have on their Individual Health Care Plan who trained the staff to administer the medication. -One did not have documented on their IHCP the side effects of the medication.

A review of staff meeting documentation indicated the following: -The month of September 2022 did not have documentation of who attended the meeting as required.
In the Infant 1 Classroom the refrigerator did not have a thermometer as required.

The Toddler 3 Classroom did not have an evacuation procedure posted at the exit as required. -The Toddler 1 Classroom did not have an evacuation procedure posted at the secondary exit in the classroom.
