A review of eight children’s records showed that four children were missing updated physicals, seven children were missing progress reports, and four enrollment packets were expired. The licensor encouraged the educator to review all children’s files to ensure that all required documentation is complete and up to date.

Records were reviewed for 10 enrolled children. Two children were missing proof of current physicals and immunizations. Two children were missing proof of current lead screening.
The educator could not locate the 2024 evacuation drill logs during the visit. She did have the logs for previous years. Please upload the 2024 drill log and document how you will have it easily accessible.
The first aid kit was missing the children’s emergency cards.
Progress reports were reviewed for the 6 enrolled children. Three children needed proof of current progress reports.
There was a metal shelf unit in the basement that was a tipping hazard.

There is one surface that is disposable for diaper changing. The other surface was fabric, making it unable to be disinfected after each use.

Progress Reports
The children's progress reports were reviewed and were not up to date.
In the kitchen there were two drawers and a cabinet that were not locked allowing access to cleaning supplies, knives and other hazardous items.
There was sitting water in a few of the play materials and a slide over 18 inches which did not have an appropriate fall zone.

A drawer in the upstairs kitchen was accessible to children and contained items which present a choking hazard, including push pins. A drawer in the downstairs kitchen contained a hot glue gun with a cord longer than 6" which poses a strangulation hazard. There were also small shelf support pegs accessible in the drawer.

The bathroom trash was not covered as is required
The Licensor observed liquor bottles accessible on top of the hutch in the dining room. The Licensor observed one bookshelf in the basement level that wasn't bolted and could pose a tipping hazard.
