The Licensee stated that the town building and fire inspectors came to the program on 11/6/25 for an annual inspection. At that time, additional work to replace the emergency light bulbs was identified to be completed. The inspectors came back on 11/14/25 and the program is awaiting the town to issue the updated fire and building inspection certificates.

Out of five children's records, two did not contain Individual Health Care Plan forms.
Staff schedule did not reflect appropriate group size and ratios.
In the front classroom, above the sink, crackers, granola bars, and muffins were not stored in covered containers. Food must be stored in clean/covered containers.
The following was observed and accessible to children on the OUTDOOR PLAY AREA: -On the exposed gutter, sharp edging was exposed and in need of repair. -The covered storage container had an unsecured lid and is in need of repair. -The top of the play kitchen had mold.
A review of the (REDACTED) indicated: -One child did not have a emergency medication available on the day of the visit.
A review of five children's records indicated: -Two records did not contain Individual Health Care Plan forms creating a possible emergency situation.
