1) The program did not maintain qualified staff to promote the health, safety, growth and development of a preschool child. 2) The program did not maintain accurate attendance as the preschool child was not listed on the attendance record.

The program did not have sufficient materials for large motor activities in the indoor gym.
The fire inspection is expired.
The depth of the impact absorbing material (wood chips) in the fall zones measured 4 inches under and around the swings, slides, and climbing equipment and did not meet the requirements.
It was observed that two exits in the gym did not have posted evacuation diagrams and procedures.
There was no evidence of a current fire inspection.
