During the visit, the Preschool C First Aid Kit was inspected. The kit did not include tweezers, scissors, or a CPR Mouth Guard.
The wooden kitchen set in the Preschool 3 Classroom and the wooden refrigerator in the Preschool A classroom were not secured to prevent them from tipping over. The plaster is chipping on the rear half wall in the PreK 4 Classroom.
At the time of the visit, one child with emergency medication was attending the program. However, the medication was not on the premises therefore, it was not immediately available if needed.
One child's Individual Health Care Plan was not current.
During the visit three staff files were reviewed. One file did not include documentation showing the completion of the EEC 12 Essential trainings.
The location of the Allergy List was not accurate in the Toddler 2 Classroom.

During the visit, two staff files were reviewed. One file did not include a Staff Form. One file did not include a resume/application. One file did not include an interview date. One file did not include a current physical. One file did not include evidence of immunity to Measles, Mumps and Rubella. Two files did not include documentation showing completion of the 12 EEC Essential trainings.
The following hazards were observed indoors: safety latches on several cabinets and drawers in the Toddler 2, Preschool A, Preschool C Classrooms were not in working order, leaving cleaning chemicals and plastic bags accessible to the children. There was rust on the lower wall heating vent in the Pre-K 1 Classroom. An educator's purse was accessible to the children in the Pre-K 2 Classroom.
In the Pre-K 3 Classroom, there is chipping paint on two window sills and chipping plaster on the half wall.
On the toddler playground, there was not the required 6 feet fall zone between the brown climbing structure and the chain link fence. There was an inadequate depth of impact absorbing material under and around the fall zones of the climbing structure located on the preschool playground.
One child's Individual Health Care Plan had expired. Another child's Plan did not include all of the required information.
Staff in the Pre-K 2 Classroom did not take a child's epi pen with them to the playground. Emergency medications must be immediately available if needed.
There was sunblock, Desitin and plastic bags on a low shelf and accessible to the children in the preschool bathroom.

In the Infant classroom, the evacuation crib was being used to store baby items. The cribs must only be used for their intended purpose.
Evacuation procedures were not posted next to the rear exit located near the Pre-K classrooms.
In the Toddler 2 classroom, the teacher did not wash the child's hand with soap and water after a diaper change.
