The licensor reviewed 4 of the children's records. The following were observed: 1.One of the children records is not uptodate. The parent last signature was 2/25/25. 2.One of the children records did not have the assistant name listed as part of the permission to transport to medical facility. 3.Two of the children records did not have a physical available. 4.One of the children records did not include the assistant name as part of the general permission to basic transportation and permission to transport to medical facility. 5.One of the children records did not list the child schedule. The record has the following listed “Rotary, schedule copy provided”. The schedule was not available. 6.Two of the children records did not have a physical description or a picture of the child.
