In the bathroom licensor observed a mouthwash on top of the sink and a plug-in oil scent that were accessible to the children. The bathroom cabinet locker was broken, and inside, safety hazards were observed, including, but not limited to, hair products, a flat iron, and a blow dryer. In the dining room, licensor observed a wired vacuum charging and accessible to the children.

There were two portable space heaters in the playroom area. The portable space heaters were not on during the visit but are used as a means of heating the room.
In reviewing the children’s records there were two children who did not have a current progress report and one child with no date on the regress report.

The first aid kit was not complete, missing the children's emergency medical cards.
The play-space room was cold, the room does not have a thermometer and the temperature was undetermined. Licensor had a jacket and licensor was cold.
There were two electrical outlets uncovered accessible to the children.
In the playroom there we’re disinfectant wipes accessible to the children. In the bathroom one of the safety locks on the door under the sink was not connecting to lock and made hazards accessible such as nail polish remover.
This investigation inspection recorded no violations or advisories.

Water temperature was 132, over the maximum limit of 120. A toy kitchen in the playroom was not secured to the wall and poses a potential tipping hazard. There are two slides measuring taller than 18" which are only partially over protective covering/fall zone. Both items require a fall zone extending 6' on all sides.
A drawer in the living room contained the following hazardous items and was accessible to children: over the counter medications, a charging cord, and cough drops that pose a choking hazard
