-In the Scallop classroom, there were rest mats stored in the bathroom adjacent to the toilet area. Rest mats must be stored in a sanitary manner. - In the Scallop classroom bathroom, there was a mop, bucket and vacuum. These items are considered to be hazards and must not be accessible to children.
Diapering and Toileting for children under 2.9
The diaper changing surface pad in the Oyster classroom had two rips/tears in it and it is in need of repair/replacement.

On the day of the visit, the posted Staff Schedule was dated 10/7/24 and was not reflective of the current staff at the program on this day. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors

At the time of the visit, the program Licensee/Director was not onsite. The individual listed as being the designated in-charge administrator next to the staff schedule on the bulletin board in the hallway was also not onsite at the time of the visit. - If the administrator is not on site, the program must have a qualified person designated in charge on site. The licensee must ensure that the program is soundly administered by qualified persons designated with specific administrative and program responsibilities.
The Staff Records were not available for review at the time of the visit. As of 11/7/24, the Licensor has not received the records for review as was requested.

The white structure with the double yellow slides did not have an adequate depth of safety surfacing as is required. In addition, the structure does not have a manufacture label indicating the age group usage for the equipment. It is recommended that the program consult with the certified playground safety inspector regarding this.
A walk through of the outside playground space indicated the following: -There is what appears to be a homemade web-like rope climbing section adjacent to the large grey and green climbing structure. There are 6 wood pilings/poles of varying heights (approximately 4-7 ft tall) that have rope intertwined and connected in many different directions and heights. The effect of this appears to be similar to a geo-dome rope obstacle for children to climb. -There is a similar secondary web-like roped area in a tree adjacent to the building itself, nearest the windows of the mixed Toddler/Preschool class. Neither of these rope-like structures are approved "climbing structures" and must be immediately removed as both are a potential safety issue for children. -There is a blue nap mat that appears to be somehow fastened to a tree on the playground next to a piece of a large tree branch that lays on the ground. It is unclear as to how and why the program has these items there and their intended purpose.

During the visit, the classroom was not prepared with all children's activities, shelving and furniture. The Licensee stated during the visit that they are in the process of arranging all classroom items and will send pictures of classroom space to Licensor for review once completed.

During the visit, there was a rope swing on the playground. The rope swing was placed between a climbing structure and a tree that had a blue and red nap mat that was hung on it. The Licensor informed the Director and the Licensee that the rope swing was not approved for use by EEC nor was there an adequate depth of impact absorbing material on the playground around the rope swing. The Licensor informed the Licensee and the Director that the rope swing must be removed immediately.

During the visit, there were two (2) large 30 gallon blue bins in the hallway adjacent to the Infant/Toddler classroom and bathroom. Exits and evacuation routes must be kept clear of obstructions. At the Licensor's request, the Licensee moved both storage bins during the visit.
During the visit, there was a guinea pig in the Director's office. When the Director was asked about whether parents were made aware of a pet being in the program, the Director said no. The program must notify parents in writing prior to the introduction of any pets into the program.

Upon a review of three staff records at the program, -Two staff did not have evidence of a physical exam, MMR, orientation, or current Professional Qualification Registry certificate. -One staff member did not have evidence of completed EEC Essentials training through Strong Start.
A review of medication stored at the program indicated that one child's prescribed (REDACTED) was not in its original container with affixed prescription label.
