The educator did not have the sign-out time for the children on 07/30/25. The educator does not have documented attendance for 07/31, 08/01, 08/04, and 08/05.

The water temperature was tested and measured at 126 degrees. The water must measures under 120 degrees.
The educator has 30 hours of ongoing professional development, but only has five of the ten required hours in diverse learners training.

The attendance records did not include a time out for one child on 06/28. The time for 6/19-6/21 there were several time in/time out not documented. On 07/03, several of the children's time in/time out were not documented.

In the outdoor space, a trampoline was observed with a ripped netting making it accessible to the children. A pool was also accessible to the children without the appropriate barricade. In the indoor space, there were various electrical outlets which were uncovered.
The provider did not have a second surface able to be disposed of or disinfected for diapering.

All of the children’s records were reviewed, one child’s general permission and emergency card were not fully completed, one child required an annual update form and one child’s enrollment packet was not fully completed.

In observing the provider’s evacuation drill log, provider has not documented an evacuation drill for the month of June and is not alternating exits on a monthly basis.

Four children's records were reviewed. Two children were missing physicals. One child did not have an enrollment packet. There were some inconsistencies with the general permission form and some enrollment packets were not filled out completely.
Provider's license wasn't posted. It was in a binder.
First aid kit was missing thermometer, scissors, and children's emergency cards. Provider added the thermometer and scissors to the kit during the visit.
Refrigerator temperature was 15 degrees. .
Provider had a fabric sheet on the diaper changing pad.
