All of the progress reports were reviewed, two children required an updated progress report.
Provider is documenting the name of one of her assistants on the attendance but is not documenting the time in and time out. One assistant's attendance was not documented on the attendance.

The first aid kit was not outside with the children and providers while they were outside.
The water temperature measured 125 degrees, over the allowed 120 degrees.

Educator is not documenting attendance for her assistants. It was observed that the educator’s own child was not listed on her attendance records.
Based on attendance records not reflecting assistants present it reflects educator being out of her licensed ratios. Attendance records reflected 9 children present on a daily basis without an assistant, due to educator not having attendance for her assistants.
This change of location visit inspection recorded no violations or advisories.
The second floor level didn’t have a carbon monoxide detector, as required.

The window opening in the play area is accessible. Please cover the openings.
One child care booklet is incomplete and one child has a physical on file that is over a year old.

The first aid kit is missing an instant cold pack.
Two shelves in the playroom are not bolted and could pose a tipping hazard. The television cord in the living room is loose and could cause strangulation.
