In the kitchen, a drawer did not have a safety lock allowing access to cords, batteries and screws. In the bathroom the drawers did not have safety locks allowing access to trash bags and makeup.
The educator does not have a completed staff record for their assistant. The educator did not have their training certificates, first aid/CPR certificate and medical form.
The evacuation drill log did not have the time of the drill documented, the length of time of the drill and the educator is not consistently alternating exits on a monthly basis.

The educator has not documented departure time for the children on the attendance log since November 4, 2025.

The educator is not documenting the attendance of the assistant staffing the program.
The following safety hazards were observed and were accessible to children: PLAYROOM: -Three electrical outlets were uncovered. -Wipes and gloves were within the children's reach. KITCHEN: -Several kitchen drawers were unlocked, providing access to batteries, sharp and pointed utensils such as knives, vegetable peelers, and openers, as well as medication, hand cream, and beauty products. -One electrical outlet was uncovered. -A bread knife was accessible on top of the kitchen island. BATHROOM: -The cabinet door was unlocked, allowing access to hair products, beauty products, air freshener, and hand sanitizer. -Cleaning products, including disinfectant spray bottles and toilet bowl cleaner, were accessible underneath the cabinet.
