At the time of the visit, the assistant {REDACTED} and assistant {REDACTED} did not have medical forms, PQR certificates, CPR, First Aid, and the training certificates in the years 2021, 2022, 2023, and 2024 in their file folders. Please upload the pictures.

Random review of children's records: Child 1 was missing permission to transport to a medical facility and administer CPR/FA. Child 2 was missing assistant’s name on permission forms, the physical was not update. Child 3 was missing physical exam, the update form and {REDACTED}. Child 4 was missing assistant’s names on permission to transport the child to a medical facility.
Licensor observed that the toy shelves in the playroom were not secured to the wall.
During a random review of children's records, Licensor observed that the assistant’s names were not listed on children’s permission forms.

Licensor observed that the baseboard heat in the playroom was missing the cover. The internal sharp metal part were exposed.
Licensor observed an uncovered outlet in the playroom. The outlet plate was broken.
Licensor observed that the screen door was not in good repair. The screen was ripped.
Exit Sign
Although the educator had a pressure gate at the bottom of the stairs leading to the lower level playroom, the gate was not properly secured on from both sides, to prevent the children from having access to the unlicensed upper level space.
Licensor observed four uncovered outlets in the playroom area.

Licensor did not observe evidence of the First Aid and CPR for the educator or the regular assistant. Note: The educator stated that the CPR and First Aid were completed and copies would be sent to Licensor via email.
Licensor observed, the following items were missing from the First aid kit, 1) CPR mouth guard. 2 ) Children Emergency Cards/Family Contact Information.
Licensor observed two progress reports checklists that were incomplete, and there was not evidence of completed progress reports for the other children.
Licensor observed that both of the emergency exits did not have “Exit Sign” posted.

Licensor observed, the educator PQR # {REDACTED} expired on 11/30/19.
Licensor observed a large TV that was not properly secured, presenting a tipping hazard for the children.
Licensor observed, the following indoor hazards accessible to the children: 1) unlocked cabinet in the bathroom under the sink. The door was tilted to one side, with siliconized acrylic chalk, mouth wash, and shower curtain metal hooks accessible to the children. 2) unlocked drawer in the bathroom vanity with a light bulb, and shaving cream inside. Licensor observed the following outdoor hazards: 1) unlocked gate that was slightly open, which provided children access to an in ground pool. 2) four pieces of rotten tree stump. 3) Four large bags of pea gravel, including a bag that was open, that presented a choking hazard for the children.
