During the outdoor play space inspection, the licensor observed the following safety hazards accessible to the children: A slide with a fall height of 21 inches and a climbing structure with a fall height of 48 inches did not have the required impact-absorbing surface underneath, and the six-foot clear space around the equipment was not maintained. A playground structure with a slide with a fall height of 17 inches was placed on cement covered with a cushioning material that does not meet EEC regulations.
