Observations from original visit on 2025-10-30 00:00:00: The infant classroom was out of ratio when two teacher assistance were left alone. The toddler classroom was out of ratio when the classroom was left alone with an E.I specialist On the day of this visit upon arrival to the program and entrance to the infant room, EEC staff observed 2 assistant teachers assigned to supervise 3 infants without a qualified educator being present.

Two first aid kits were missing scissors and a CPR mask. One of those bags was also missing tweezers and a thermometer.

During the visit, the following supervision issues were observed: 1. Preschoolers were left unsupervised for a period of time when the teacher left the room to answer the doorbell. 2. A preschool child was found alone in a supply closet while eating a lollipop. 3. Two toddlers were left alone in the classroom with an Early Intervention (EI) specialist when the teacher went to answer the front door.

Review of educator files revealed that a new hire lacked the required essentials prior to employment.

A review of a selection of staff records revealed the following findings: 1. Two files were missing at least one reference check. 2. Two PQ registrations had expired. 3. One file did not include proof of the required MMR vaccinations. 4. One file contained an expired First Aid and CPR certification.
A review of children's files revealed the following findings: 1. Two first aid/emergency consent forms were missing dates. 2. One first aid/emergency consent form was missing a signature. 3. One first aid/emergency consent form was missing required information.
Review of Educator Files: One staff member had an expired first aid certificate.
An infant was observed drinking from a bottle in a crib.
A review of attendance records revealed that the toddler classroom did not have their attendance readily available.
The following safe sleep violations were observed: 1. An infant was found sleeping on a loose sheet. 2. All cribs were noted to have loose sheets. 3. Gaps were observed between the mattress and the crib wall, and one crib mattress was folded. 4. An infant sleeping in a crib had a binky in their mouth that was clipped to their clothes with an attachment.
On the day of the visit, current staff schedule was not posted.
The arrival time of an infant was not documented on the attendance.
An infant was not held while being fed, they were drinking from a bottle in the crib. The infant feeding schedule was incomplete.
A garbage receptacle was observed to be uncovered.
Several infant bottles were not labeled with children's names.

The following safety issues were observed: 1. A teacher's purse was left accessible to children. 2. A cord was not properly secured, leaving it accessible to infants. 3. In the toddler classroom, both the phone cord and the iPad charging cord were accessible to children. 4. The preschool's phone cord was also accessible to children.
During the visit, a current building inspection was not available for review.
This provisional license renewal visit inspection recorded no violations or advisories.

There was inaccurate attendance. There were 4 children present but only 3 written in on the attendance tracking form. This is the third time that this non-compliance was cited. It was previously cited on 3/5/25 and 3/12/25.

The program had an infant when they are not licensed to serve infants nor is it approved on their building inspection. This was previously cited on 3/5/25.
EEC observed the Toddler/preschool classroom was only staffed with 2 teaching assistants.
During the safe sleep follow up visit, EEC observed a second infant with a blanket on them in a program that is not licensed to care for infants.

None of the children enrolled had physicals. Many of the children enrolled had inadequate child files without all required documentation either through paper files or through electronic files as none of the program's computers were set up/ plugged in or had Wi-Fi, from what EEC observed and according to the program staff.
One staff in ratio with children did not have a background record check (BRC) as required by regulation.
There was inaccurate attendance, EEC observed 7 children in care and only 4 were signed in.
Even though the program currently is not licensed for an infant grouping, a 9 month old infant was in the Toddler Preschool grouping and EEC observed the 9-month-old in a porta crib with a blanket on them which is a safe sleep violation.
The program is currently only licensed for a Toddler/ Preschool grouping of 9 children. EEC observed a 9-month-old infant sleeping in the T/PS classroom. In addition to the program not being currently licensed for infants the City of Boston Building Inspection clearly states on in "No Infants".
