In the Toddler 1 classroom, there was a tall plant stand that was tippable.
One child had 2 medications. Each medication did not have a completed Medication Consent Form.
1) One child's Individual Health Care Plan included the use of medication for a mild reaction. The parent has not provided the medication to the program. 2) A medication dosage dispenser for the specific dosage amount did not accompany the liquid medication.

A sampling of children's files was reviewed. One child's Medication Consent Form did not include directions for storage.
1) In the Preschool 2 classroom, the edge of a rug was curled up and posed a potential tripping hazard for children. 2) In the Toddler and Preschool shared bathrooms. there was a toilet bowl brush accessible to children.

There was some peeling/chipping paint on the wall in the shared Toddler bathroom.

In Infant 2 classroom, there were 2 electrical outlet that were not covered. Electrical outlets within the reach of children younger than school age shall be made inaccessible by use of a safety device or covering that prevents access to the receptacle openings.

Two staff who have been working at the program for 6+ months did not have evidence of current First Aid Certification.

Written emergency and evacuation plans were not posted at each exit. The plans should include where the meet upon evacuation from the program.
EEC observed children in Toddler 2 and Preschool during naptime. Teachers informed EEC that they had been on their mats for approximately 45 minutes. Several children were on their mats and awake. These children who chose not to sleep or awaken early, were not offered quiet activities for the remainder of the sleep or quiet activity period.
1) One child's Enrollment forms were not signed/dated by the parent. 2) One child's file did not include evidence of a current Lead test and results. 3) Two children's files who have medication did not include a Medication Consent Form completed and signed/dated by the parent per medication.

1) One staff file reviewed did not have evidence of 2 reference checks. One staff file reviewed did not have evidence of a 2md reference check. 2) One staff file reviewed did not have evidence of a current physical exam. 3) Three staff files did not have proof of 2 doses of MMR or proof of immunity. 4) Three staff files did not have evidence of bi-monthly staff observations.
1) When EEC walked into Toddler 1 classroom, there were 9 children present and 1 teacher. The 2nd teacher had stepped out of the classroom to go to the kitchen. This classroom did not maintain proper staff:child ratios as all times.
On the day of the visit, there were 16 children napping in the playroom. This room's square footage allows for a maximum of 10 children. There should never be more than 10 children in this classroom.
Written Emergency Phone Numbers did not include the phone number for Poison Control.
1) The evacuation drills for the last year all took place within a 15 minutes time frame (3:30PM-3:45PM). The program must practice evacuation drills during all hours of operation.
1) One child's medication was not in the original box with prescription label including the child's name.
Two children's Individual Health Care Plans did not include consequences should the medication not be administered signed/dated by the child's doctor.

Outdoor IPM Plan
The outdoor Integrated Pest Management Plan (IPM) was over a year old. IPM Plans must be reviewed and updated yearly. Please email EEC when a current copy of the outdoor IPM Plan with required signatures is uploaded to the document library
There was a 6 seater buggy in the hallway which was an exit obstruction. All exits and evacuation routes must be kept clear of any obstructions.

One child's file reviewed included that they had allergies (no medication required). One child's file included that they had a sensitivity to a food group. The information for both of these children was not noted on the Allergy/Medical Condition List.
1) The program Health Permit has expired. 2) The program gets food delivered from Happy Kitchen and their health permit has expired.
