The allergy list was not posted in rooms 110 and 109. There were children on medications and they were not listed.
The emergency evacuation log was missing the months of October, December, and February.
A review of staff records, Licensor noted that two staff files were missing evidence of staff observations.

1. The staff meeting documentation for March and June were for 30 minutes from 7:30 - 8:00 a.m. 2. Staff meeting documentation for April was for one hour.
At the time of the visit, UPK 1 room had 20 children listed on the attendance but 16 children were present. 2. On 9/18/24, only one child was signed out on the attendance roster in UPK 2 room.
At the time of the visit Licensor observed a teacher assistant taking 8 children from the UPK class to the bathroom alone.

There was insufficient evidence in the file to determine that the program administrator was qualified.
One staff had an expired physical examination dated 1/3/2020.
During the visit, Licensor observed four children in the hallway alone putting away their blankets without adult supervision.
Three children were missing the parental consent form for the emergency medication. 2. One child with two emergency medications did not have a parental consent form for each medication.
1. One child had an expired medication dated 1/2024. 2. One child on an emergency medication did not have the medication at the program.
1. Two children on an emergency medication had an incomplete IHCP missing the potential side effects of treatment, and potential consequences if treatment is not administered and it was not dated.
