A sampling of medication was reviewed and one medication was expired, along with the prescription label

The staff meeting log was missing proof of a meeting occurring in June and July of 2022.

A sampling of Individual Health Care Plans were reviewed and the following was found: 1. Two plans were missing who provided the training 2. One medical consent form was missing the dosage, when the medication is to be administered, and the directions for storage.
One medication was not appropriately labeled; the original prescription label was not affixed
Clear containers containing snack did not have expiration dates on them in the Infant/Toddler room and the Toddler/Preschool room.
In the Toddler 2 room and the Infant/Toddler room, evacuation procedures were not posted on each exit.

In the Infant classroom, a child was observed being fed a bottle while in a bouncy.
Three staff files were reviewed and the following was found: All three files were missing proof of verification of two references.
The staff schedule was not posted in an area easily visible to educators, parents and visitors.

The Children's Record Checklist and three children's files were reviewed and the following was found: 1) One child's First Aid and Emergency Medical Consent Form was expired. 2) One child's Transportation Plan was expired. 3) Two children's files did not have copies of incident reports.
One infant child's file was missing evidence of a 3 months progress report.
The Health Care Policy does not include all required information.

The Infant 1 and Infant 2 classrooms were combined because of a low attendance numbers. Children must be assigned to a consistent group with consistent staffing.
Three children's Individual Health Care Plans (IHCP) were reviewed and the following was found: 1) All three IHCP's were missing who provided and who received training on medication administration. 2) Two IHCP's were missing the possible side effects if medication is not administered.
A child's prescription medication was not in the container in which it was originally dispensed in. The medication was missing the original labels affixed.
