At the time of the visit EEC observed an educator on their cell phone while out on the playground with children not supervising the children and/or following the program's cell phone policy.

In several of the classrooms the children's food was not being stored properly and was scattered throughout the classrooms. The program did not properly wash the children's cups and dishes after each use as the sink had them in it and the drying was dirty.

The {REDACTED} classroom had several chairs that were broken, not appropriate size for some of the children in the classroom, and several chairs were stacked on the sensory table while the children were in the classroom.
The program had several cabinets and shelves that need to be secured in a manner that the furniture will not tip over.
The program was not using the crib as the intended purpose as a bouncy seat was being stored in the crib a child had just slept in that day.
At the time of the visit the program had several wires longer than 6 inches, several purses, a basket of sunblock on shelf, and some cleaning supplies accessible and within reach of the children.
The program did not have a enough cribs for the amount of {REDACTED} enrolled in the {REDACTED} classroom as there were only three cribs and six infants.
At the time of the visit one crib had a child sleeping on double mattresses instead of one mattress and (1) crib sheet was too big for the mattress.
At the time of the visit the program had a second safe sleep noncompliance with one child was asleep in a bouncy seat not moved to a crib and a second child was sleeping on a mattress with a pacifier attached to their shirt by string with beads.

At the time of visit EEC observed that the attendance record in one classroom was not accurate. The number of children listed on attendance record did not match the number of children present. In another classroom the educator was not fully aware of their daily count as the educator proceeded to count the children's lunches on table when requested to report their daily attendance.
2 refrigerators did not contain thermometers.
EEC observed the following potential hazards at the time of the visit: 1) Electrical cords accessible to children in multiple classrooms. 2) Bottle warmer in the infant classroom. 3) Sharp knives stored in an unlocked drawer. 4) A hammer and plastic bags stored in an unlocked cabinet. 5) A garbage container without a cover. 6) Educators' bags/purses accessible to children.
At the time of visit the updated fire inspection was not available for review.
At the time of visit program director was only able to provide EEC with documented fire drills for August, September and October. There was no evidence that monthly fire drills are being conducted consistently.
At the time of visit EEC observed the following personal items not labeled and/or properly stored: 1) Several bottles and sippy cups/ water bottles not labeled with the name of the child for whom they were intended. 2) Several cots/mats stacked on top of each other with blankets/linens on them. The cots/mats were not being stored appropriately to ensure safe and sanitary storage of blankets and bed linens.
