During the safety inspection of the program's bathrooms, a hole was found in the wall next to the toilet.

During the safety inspection, the following was observed: 1) In the Tadpole Classroom, dramatic play equipment needed to be secured. 2) In the Tadpole and Butterflies/ Dragonflies Classrooms, chipping/peeling paint was found. 3) Water damage was found on the wall by the sink in the children's bathroom.

Three children's records were randomly selected for review, and the following was learned: 1) 3 children did not have evidence of yearly lead screening. 2) 1 child's transportation plan was not filled out correctly; it did not show how the child arrived and departed. 3) 1 child's Face sheet was not signed or dated by the parent.

During the review of the fire drill log, it was found that the program did not complete drills in November or December 2022.
One of the Educators did not complete the two-step cleaning requirements.
At the time of the visit, not all of the children's water bottles were labeled in the Dragonflies classroom.
