Outdoor play space
A sampling of staff files were reviewed. Three staff files did not include a current bi-monthly observation.
At the time of the visit, the outdoor tennis courts did not have any shaded areas. The outdoor play area must provide for both direct sunlight and shade.
Fall zones adequate depth of impact absorbing material
The tennis courts had large green sheets of material hanging on two sides of the fence. The condition of this material posed potential entrapment hazards. 1) On one side of the fence, the hanging sheet of green material had torn away from the fence on one side and the material hung to the ground. 2) On the other side of the fence, the hanging green sheet material had many slices throughout the material.
Samplings of the depth of ground cover in and around the use zones on the playground structure and the elevated balance beam found that the depth is under 9". The depth of ground cover in and around the use zones must be at least 9".

Supervision at the tennis courts
On the day of the visit, EEC observed 2 preschool groups of children playing outside at the tennis courts. Children were playing throughout the entire area within the tennis courts. EEC observed three staff members standing together talking at the far end of the tennis courts. Staff gathered together at one end of the tennis courts did not show that staff were exerciseing appropriate supervision of the children in their care in order to ensure their health and safety at all times.

One child's Individual Health Care Plan did not include the staff who were trained to administer the medication and who trained them, signed and dated by the child's parent.
Several monthly staff meeting documentation did not include the beginning and end time of staff meeting. EEC was unable to determine if all staff were meeting for a minimum of 2 hours monthly.

1) Review of one child's file found that their Children's Enrollment Form was signed/dated by the parent over one year ago. 2) Review of one child's file found that one of the child's medications did not have a complete Medication Consent Form. Review of this child's other Medication Consent Form found that the dates to be give and the times to be give was blank.
In the Preschool playground, the use zones under and around the climbing structure was not covered with an adequate depth of an impact absorbing material, in accordance with EEC policy.
Review of the past years Evacuation Drill Log noted the following:' 1) Only primary routes were being practiced within the past year. Programs must practice alternative routes. 2) Within the past year, the latest time of an evacuation drill was 12:15 PM. Drills must be held during different times of the program day.

Weekly snack/meal menu made available; Previous menus on file for one year
A sampling of staff files was reviewed. 1) Two staff files did not include 2 doses of MMR or proof of immunity. 2) Three staff files were missing a current bi-monthly observation.
The Allergy/Medical Condition List did not include one child's allergy to tree nuts, walnuts, and pecans. This child did have other food allergies which were noted.
A sampling of menus was reviewed. The menus did not include the Pizza Friday options for parents who chose this lunch for their children.
In the Infant 1 classroom, there was a child who was sleeping, Their pacifier had fallen out of their mouth and was lying on the fitted sheet. Staff did not remove this pacificer from the crib once it fell out of the child's mouth.
The evacuation route from Infant 2 classroom included a plastic activity table that was in the middle of the evacuation route. Also, there was a door and some storm windows leaning up against the wall in the evacuation route that were not secure.
There was a large amount of sand on the poured rubber surface of the playstructure area. The large amount of sand made the poured rubber surface very slippery and a possible hazard to children.

One child's Individual Health Care Plan included the administration of an emergency medication for a minor allergic reaction. The parent had not provided this medication to the program.
