A review of staff records found the following: 1 Three educators were missing evidence of bi-monthly observations. 2. One educator was missing evidence of a physical examination.
1. One child was missing a progress report. 2. One child had enrollment forms that was missing parent signature and date. 3. One child had expired forms for transportation, first aid medical consent, and off site permission dated 7/18/23. These forms are annual. 4. One child had expired first aid medical consent forms, transportation, off site permission dated 2/27/24.
Three staff hired more than six months were missing evidence of first aid.

The staff meeting documentation included was missing the months of January, February, and June.
The evacuation drill log was missing the months of March, April, May and June.
One child had a First Aid Medical consent form dated 4/13/23, & a Developmental History, transportation form, and off site permission form dated 3/10/23. 2. One child First Aid Medical consent form and transportation form dated 4/2022.
A review of four staff files, Licensor noted the following: 1.One staff had an expired PQR dated 10/2023. 2. Three staff observations were dated 10/10/23 and not being done every two months per EEC regulations. 3. One educator recently hired was missing verification of 2 references.
