Licensor reviewed children's records and there were several missing annual update forms and physicals. Please review all records and ensure they are all updated as needed; confirm once completed.
Evacuation log was missing May, June and July drills. Please ensure evacuations are completely monthly and documented as needed.
There was some chipping paint on the deck. Please ensure there is no chipping or peeling paint anywhere on the front or back deck. Please confirm once completed and follow up visit will be scheduled.

There was excessive chipping paint on the deck/porch and stairs leading to porch. Please provide a plan on how chipping/peeling paint will be resolved.

Educator gave Licensor a virtual tour of the home and there was only one hazard noted. There was an outdoor plastic slide in the indoor play area. Please note that outdoor play equipment should not be placed in the indoor licensed space. Please remove it or make it inaccessible to childcare children.
