At the time of the visit, in a review of three students files, it was identified that Student B and Student C had expired Developmental History packets. The program must maintain students files as outlined by the staff record checklist.
At the time of the visit, it was observed that the program was storing a Swiffer mop and the broom and dustpan by the classroom sink. It was also observed that above the classroom sink, two ceiling tiles had water stains. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter.
At the time of the visit, it was observed that one educator left the classroom, to move a crib into a different classroom. This left the remaining educator in the classroom out of ratio for roughly 10-15 seconds. There must be a minimum of two educators present for a group size of seven to ten students.

This investigation inspection recorded no violations or advisories.

Staff records were not available for review during the visit. When discussed, the Licensee stated that they were "locked upstairs in the office."
The following were observed and accessible to children: FRONT ROOM - Along the wall, there was a hanging light fixture cord approximately 6 inches not secured. KITCHEN - In an unlocked bottom drawer next to the refrigerator, there was yarn approximately 6 inches that could cause a potential choking hazard; In an unlocked drawer left of the stove, there was a screwdriver and a metal screw.
The following were observed outdoors and accessible to children: There were rusty protruding bolts on the gate (previously cited on 9/19/19). There were large pieces of mulch and sticks mixed with the ground surfacing (previously cited on 9/19/19). There was a brown metal gutter with protruding sharp nails on it approximately 6 feet long in the yard.
A written diapering and toileting procedure plan was not posted in the bathroom.
The location of the First Aid kit was not posted.
It was observed that emergency medication for a child with a diagnosed allergy was not readily available in case of a potential emergency.
In the kitchen on the lunch box shelf, there were two cups not labeled with the name of the child they were intended for (previously cited on 9/19/19).
In the bathroom underneath the changing table, there was an open box of gloves accessible to children.
