At the time of the visit, the Licensor observed a child who was identified by the Director/(REDACTED), who was not enrolled in the program. The program is not currently licensed to care for toddler aged children. At no time shall a program admit, supervise or provide care for more than the maximum number of children indicated on the license.

During the visit, two staff records reviewed indicated the following: -Two did not include evidence of EEC Essentials 2.0 training which was required to be completed by July 1, 2025. This training consists of Emergency Response, Food Safety, Hazardous materials, Intro: First Aid/CPR, Intro: Child Growth & Development, Medication Administration, Missing Child Prevention, Physical Premises Safety, Preventing Shaken Baby Syndrome, Prevention & Control of Infectious Diseases, Protecting Children from Abuse & Neglect, Safe Sleep Practices, Transporting Children Safely.
During the visit, two Staff Records reviewed indicated the following: -Two staff records did not include evidence of bi-monthly observations.
During the visit, two children were left alone unsupervised as described in this report. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
The program had a toddler aged child in the Director/Owner's office at the time of the visit. There was no documented attendance indicating the child was present. The licensee must maintain sufficient numbers of qualified staff to promote the health, safety, growth and development of each child. Assignment of staff must take into account the physical environment, requirements of the activities children are engaged in, and the developmental levels and behavioral traits of children in care.
The program did not have a Children's Record for an identified toddler aged child observed to be in the Director/Owner's office at the time of the visit. The licensee must maintain an individual written record for each child.
At the time of the visit, the Director/Owner was observed by the Licensor leaving an unenrolled toddler aged child alone unsupervised in the office on two occasions; once for a period of 2-4 minutes and a second time for a period of approximately 5 minutes. The Director/Owner was observed to leave another preschool aged child alone in the preschool class unsupervised for a period of approximately one minute. The program must have evidence of the ability to provide appropriate supervision of children and staff/child ratios.
Individual Health Care Plan (IHCP) - R Apr 2025
During the visit, a substitute teacher working in the program did not have a BRC affiliated with the program. The individual is suitable in the BRC Navigation system under a FCC location of care.
Upon a review of three medication consent forms at the program, the following was identified: -One child's record did not contain a current and complete medication consent form as it was not signed and dated by the child's parent.
-During the visit, there was a substitute teacher working in the classroom. This individual did not have a staff record or any EEC required paperwork completed. The Licensee stated this was the first time the educator was working at the program. -One out of four staff records reviewed did not include evidence of MMR immunity.

The program did not notify EEC of a child injury that occurred on (REDACTED) that resulted in a child having (REDACTED) while in the care of the program.
This capacity increase visit inspection recorded no violations or advisories.
The following hazards were identified during the visit: -Classroom 2 is a licensed space, but there was no barrier between the Director's office space and Classroom 2 as required. The following hazards were accessible to children in this space: -There were wires longer than 6 inches for a laminator, a microwave, the phone, printer, computer, and a circular globe. -Two adult pairs of scissors were on the Director's desk. -A Keurig machine located on a low shelf was on. -A vacuum, a mop and a Swiffer with cleaning solution in it were to the right of the door. -Two trash containers had no lids. -Four bags/pocketbooks were on the floor near the Director's desk and door. -Four large cabinet doors were closed, but not locked allowing children to easily open doors. In the cabinets, there was a gallon of vinegar, a roll of trash bags, and several blue plastic bags.
The program had a posted "Allergy" list. However, two individual children (REDACTED) who each have an IHCP and prescription medication on site were not listed. EEC requires that a list of all emergency or life saving medications, including but not limited to (REDACTED), specifying to which children they belong must be posted in the program.

Upon a review of two Staff Records, the following was identified: -Two records did not include a current physical and documentation of an MMR vaccine.
Upon a walk through of the facilities, the following was identified: -On the white shelf near the "world" display, there was a clock/speaker that had a wire longer than 6 inches accessible to children. -There were several window blinds in the classroom that were bent and ripped that are in need to repair. -There was a tippable play "Levi and Friends Cookie Shop" that must be secured.
In the bathroom there was a toilet cleaning brush and a plunger accessible to children.

The Children's Record Checklist was not available for review at the time of the visit. Upon a review of three children's records, one child's record did not have evidence of a current physical exam for the child.
-Upon a walk through of the Preschool classroom, there were several tippable pieces of furniture (pink kitchen, red and blue sensory tables, doll house, grocery store).

During the visit. the Licensee stated that the program has staff "electronic meetings." When asked more about this, the Licensee stated that they send electronically topics for educators to read. The Licensee stated that it is challenging for all educators to find a common time to meet. The Licensee must follow the program's written plan and must document regular staff meetings of at least two hours per month to consult with educators on program issues; program planning; policies and procedures; parent communication; implementation of the regulations; children's behaviors and meeting the individual needs of children.
Upon a review of three children's Individual Health Care Plans, the following was indicated: -One child with a diagnosed chronic medical condition did not have an IHCP in their record. -One child's IHCP was not current and complete as it was not signed and dated by the child's parent.
Upon a review of three medications stored at the program, the following was identified: -One child's medication had expired in October of 2024. In addition, the medication itself was not stored in its originally dispensed box with affixed prescription label.

There was an open container of cream cheese next to the microwave near the Director's desk. There was evidence of what appeared to be crumbs from food on top of, and around the microwave as well.
During the visit, there were 10 children with two educators in Classroom 2 (205 square feet), which is located in the same space as the Director's office. Although this is EEC licensed space, there must be a minimum of 35 sq ft per child. This means there should be no more than 5 children in this space at any given time.
The program's Fire Inspection Certificate expired on 6/9/23. During the visit, the Licensee stated that the fire department had already been out to conduct the inspection but the program has not yet received the certificate.
A review of medication stored at the program indicated the following: -Two individual children's medications were not stored in its original container with the affixed prescription label.
Two children with allergies indicated on their enrollment paperwork did not have an Individual Health Care Plan on file.
Upon a review of medication stored at the program, the following was observed: -One child's prescribed ointment for (REDACTED) had expired in 2/2022. -One child's (REDACTED) was stored in a previously opened container.
