Educator had an up to date evacuation log and has conducted a fire drill every month. However, the exit used during the drills were not alternated.
Individual Health Care Plans
A child in the program has [REDACTED] a but does not have an IHCP. Educator stated the child was retested and it was determined that the child is no longer [REDACTED]However, there is no documentation of this in the child's record.

Exit Safety
Licensor randomly reviewed 3 of the children's records. Of these, 1 had out of date medical information (physical and immunizations), 1 was complete and up to date, and 1 had out of date immunizations records.
There are 2 exits from the basement space, the back door and the stairs to the 1st floor. Both exits were easily accessible. However, the exit up the stairs did not have an exit sign.
Stairways
There was no gate at the bottom of the stairs leading from the first floor to the second floor.
Educator has remodeled the entryway in the basement and would like to add this room to her licensed space. The only hazard that was observed was the access to the boiler. Educator has a gate up, but it is right up against the boiler, so the children still have access to it. Discussed ensuring that the gate set up further out from the boiler, so the children can't touch it.

The boiler in the basement entry room is exposed. The boiler needs to be made inaccessible to the children.
All of the required emergency numbers/information is posted on a bulletin board in the basement, except the nearest emergency medical facility.

Space (health risks) - R Apr 2025
The doors to the laundry were open during the monitoring visit and there was a bottle of bleach on the washing machine, which would have been accessible to the children if they were in the naproom.
Licensor walked through the licensed space and observed the following hazards: ** Kitchen - drawer next to fridge has a child safety latch that isn’t working and there are hazards inside (scissors), silverware drawer also has a child safety latch that isn’t working and there are sharp knives inside, ** Diningroom - outlet not covered ** Livingroom - no hazards ** Bathroom - child safety latch under the sink isn’t working and there are cleaners inside ** BASEMENT - ** Naproom - bleach on the washing machine is accessible to the children ** Playroom - no hazards
