At the time of the visit, documentation of an April 2025 staff meeting record was available for review.
At the time of the visit, three staff members reflected an expired background record check status as of 8/23/2025, 10/6/2025 and 10/19/2025. Background record checks must be renewed prior to expiration date.
1. A child's prescription medication (Epi Pen) was expired as of 3/2026. 2. A second prescription medication (aminocaproic acid) was outdated, reflecting discard date of 9/8/2025.
At the time of the visit, a current consent for a prescription medication (inhaler) was not available for review.
Two plastic storage cabinets approximately six feet tall were not safely secured inside a restroom and inside a classroom space used by children.
