Licensor inspected all of the indoor, licensed space, as well as the outdoor playspace (fenced in front yard). No hazards were observed in any of the indoor space. However, outside in the mulched area, there was a plastic slide that was on the edge of the fall zone. Regulations state that the fall zone needs to extend 6ft in every direction from the slide.

Licensor randomly reviewed 3 of the children's records and observed the following: ** ONE ** - out of date signatures and out of date physical ** TWO ** - out of date physical and immunizations ** THREE ** - out of date lead testing. Please review all of the children's records and ensure they are all complete and up to date.

All of the required items were observed in the first aid kit. However, one of the children’s emergency contact forms was out of date.
Educator installed a fall zone for 2 plastic slides that are over 18 inches. One slide is only 3 feet from the edge of the fall zone and needs to be moved.

Licensor reviewed the 2 children's records, 1 was complete and up to date, but the other had out of date signatures in the enrollment packet and out of date medical information.
Educator does not track the time of arrival or departure on a daily basis for the children. Attendance is also not tracked in a timely manner, the same day. Discussed that these changes need to be made to attendance moving forward.

Licensor reviewed all 3 of the children's records. Of these, 1 record had no physical or immunization information, 1 record had no physical/immunization information and had out of date signatures for the enrollment packet, and 1 record had out of date signatures.
Educator's CPR/1st Aid Expired July 2023. Educator informed Licensor that she is already enrolled in a recertification course on 11/11/23.

All of the required items were observed in the first aid kit. However, Educator needs to add the children's emergency contact information to the kit.
Educator states she has completed several hours of professional development over the last year. However, she did not have any certificates.
This follow-up visit inspection recorded no violations or advisories.
The water temperature was 130F degrees. Regulations state this temperature must be under 120F degrees.

Educator completed 10 hours of professional development over the last year. However, none of these trainings dealt specifically with early education. Discussed that because of this, additional hours of training will be required.
Licensor inspected all of the licensed, indoor space during the progress visit, as well as the outdoor playspace. The following hazards were observed: **KITCHEN - disinfectant wipes on the counter **LIVING ROOM - no hazards **BATHROOM - no hazards **OUTSIDE - 2 small slides that do not have a fall zone and the side area attached to the shed that had hazards inside and was accessible during visit.
Educator did not have an emergency evacuation log. Educator stated she has been completing fire drills, but did not have this information documented.
Licensor inspected all of the licensed, indoor space during the progress visit, as well as the outdoor playspace. The only hazard that was observed was in the bathroom. There were bags of septic saver tablets on a low shelf.
An 8 month old infant had a blanket and toys in the bassinet with her while going down for a nap. Another infant typically sleeps in a swing.
Educator reported that the children bring extra clothing in their backpacks/diaper bags daily. Discussed keeping clothes at the program for the children. The children's bags also need to be stored so that they do not touch each other, to avoid cross contamination.
