The provider did not follow EEC Safe Sleep Policy or the manufaturer's guidelines regarding the crib set up in the program for the (REDACTED). At the time of the visit, The provider did not have any other adequate crib or bassinet available for the infant, who fell asleep, to sleep in. During the visit, a crib from the provider's home was brought into the childcare space and was set up for the infant.

There was a moldy/musty smell in the childcare space. In addition, when the crib was removed from the space, visible mold was observed on the bottom of it.
The crib used for the (REDACTED) did not have a proper mattress. The provider placed a 3 inch mattress in the crib that was not the original mattress provided by the manufacturer. EEC policy and the safety certificates on the crib stated that only the mattress that came with the crib should be used and it must not be more than 2 inches thick.
The wood diaper table did not have a diaper pad that was smooth, and impervious to water and easily cleaned. The table surface was in need of cleaning.

There were rusted paint cans and trays on the fence in the outdoor area used for play that needed to be removed/replaced.

The safety latch for the cabinet under the bathroom sink was not working. There were cleaning products under the sink.

The bathroom floor was in need of thorough cleaning under and around the diaper table.
Upon arrival the provider was holding a (REDACTED) that fell asleep in her arms during the visit. There was no adequate crib set up for the infant. The crib mattress had a plastic cover that had to be removed during the visit prior to the provider placing the infant in the crib. There was no fitted sheet on the mattress. Crib sheets in the other cribs children were sleeping in did not adequately fit the crib mattresses and were attached to the corners of the cribs with elastics.
