General: Based on observation, the surveyor found six (6) cribs in the Nursery that contained loose fitting crib sheets; therefore, the cribs sheets were not tight-fitted. The surveyor observed one (1) child who was asleep and one (1) child who was awake placed inside two (2) of the cribs. Through interview with staff, the surveyor learned that the child who was asleep was approximately ten-months-old and the child who was awake was approximately three-months-old. General: Based on observation of the preschool room, one (1) child's (DOE: 6-29-16) first and last name along with a list of their known allergies was posted on a bulletin board. The bulletin board was located next to the front door of the classroom and did not contain a cover page; therefore, confidentiality was not maintained as it was visible to anyone that entered the classroom. Based on interview with the director, the child was drop-in status and was only present for two (2) days this year. The child is no longer enrolled at the childcare center.
