General: Based on observation in the toddler room, there was a pre-measured syringe in a child’s lunch box containing "Bethanechol". There was no label indicated drug facts or the name of the medication on the syringe. The medication syringe did not have a child’s name listed on it, nor was it in a locked container. General: Based on review of documentation, this regulatory requirement was not met. One (1) staff file (hire date: 8/21/17) contained results of a negative T.B. test that is no longer current. General: Based on review of documentation and interview, this regulatory requirement was not met. A review of staff files during the inspection found twelve (12) staff that did not have the date of employment listed in their file. The staff person in charge reviewed the background fingerprint date documented on each staff person's form and then verbally identified the dates - 3/6/19, 9/13/19, 5/13/19, 8/19/19, 2 on 8/20/19, 2 on 8/23/19, 8/21/19, 2 on 8/29/19 and 9/3/19 as the hire dates. However, the dates of hire were not maintained in the personnel files. It is important to note that the date identified as the hire date by the staff person in charge was not the same as the date documented on the background fingerprint form in a few instances.
General: Based on review of documentation, and the Training Records Information System (TRIS), the following was found: 1. One (1) staff person did not complete the six (6) hours of cabinet-approved training within the first three (3) months of employment. The staff person was hired on 3/6/19; however, the staff did not complete the training until 7/31/19. 2. One (1) staff file (hire date: 5/13/19) did not contain evidence of the six (6) hours of cabinet-approved training being completed within the first three (3) months of employment. The staff person in charge acknowledged during the visit that the staff person had not yet completed this training. General: Based on Review of Documentation, it was found that the file and ECE-TRIS record for an employee hired on 1-15-16 did not contain evidence of the six-hour cabinet- approved orientation training being completed. 345 - Driver Requirements Not Applicable 922 KAR 2:120. Section 12. Transportation. (18) A driver of a vehicle transporting a child for a center shall: (a) Be at least twenty-one (21) years old; (b) Complete: 1. The background checks as described in 922 KAR 2:110; and 2. An annual check of the: a. Kentucky driver history records in accordance with KRS 186.018; or b. Driver history records through the state transportation agency that issued the driver’s license; (c) Hold a current driver’s license which has not been suspended or revoked during the last five (5) years; and (d) Not caused an accident which resulted in the death of a person. General: Based on Observation, it was found that an apartment building owned by the University of Kentucky created one of the boundaries of the playground. The wooden casings of the bottom windows of this building were observed to be rotting and badly splintered. The paint on these casings was also observed to peeling. During the survey, children were observed to be playing by the windows. Two (2) children were observed to running their fingers along the casings. The director stated that she submitted a work order for the windows on 2-25-16, but they have yet to be repaired.
General: Based on Review of Documentation, it was found that one (1) child's file out of ten (10) children's files reviewed did not contain an immunization certificate.

A staff person hired 9/3/15 had listed an address in Ohio as a previous residence in the past five (5) years. There was no evidence of a background check from Ohio for the staff person on file in the facility. 4. A staff person hired 8/19/15 had listed an address in New York as a previous residence in the past five (5) years. There was no evidence of a background check from New York for the staff person on file in the facility.
