General: Based on observation, the following was found during inspection: Preschool 2 Classroom 1) There were areas of chipped paint by the circle time area. 2) The wall beside the sink was excessively dirty with stains and discoloration. General: Based on interview, an injury that required medical care was not reported to the Division of Regulated Child Care. A child was sent home at the end of the day, and a parent sought treatment for an eye injury that occurred on the playground during care at the center. Director Requirements In Compliance
General: Based on observation, the following was found: Toddler 1 Room 1.) Several cables/cords were plugged into an extension cord that was set on the corner of the children's table and was accessible to the children in care. Preschool 1 Room 1.) Children’s coats were touching as they were hanged on the hooks close together. This method of storage is unsanitary and promotes the spread of germs and bacteria. Preschool 2 Room 1.) General: Based on observation, a staff members styrofoam cup was sitting on the edge of a shelf in the Toddler 2 Room.

General: Based on interview and review of documentation, the surveyor found that 04/16/24 the Toddler #1 classroom was over-ratio by three (3) children for at least an hour. The staff member had nine (9) children ages twelve to eighteen (12-18) months. Interviews found that a staff member called into work unexpectedly; therefore, the Toddler #1 and Toddler #2 classrooms had to combine. General: Based on interview, on 12/22/23, a three-month-old child was given the wrong breast milk. The child took the whole bottle and the teacher did not noticed the mistake until she was cleaning the empty bottle. Parents were notified and child was monitored. General: Based on interview and review of documentation, the surveyor found that on 7/13/2021, a two-year-old child fell on top of an infant, and attempted to bite the infant. The staff person, who was sitting on the floor next to the infant, yanked the two-year-old child to the floor by his arm, and pulled the child behind her across the carpet to prevent him from biting the infant. The two-year-old child cried, and received some redness to his knees; however, the redness quickly disappeared. The staff person expressed that she did not mean to harm the child.
