General: Based on observation, the child care center failed to maintain state regulations in regards to the premises. Observation of the boys' restroom revealed a section of drywall underneath a toilet paper dispenser in the second stall was damaged, exposing the mesh netting that was protruding outward. General: Based on review of documentation, the child care center failed to maintain state regulations in regards to employee records. General: Based on review of documentation and interview, this regulatory requirement was not met. The surveyor reviewed 10 children's files during the visit and found the following: 1. Nine did not contain authorization by the parent for the facility to seek emergency medical care for the child. 2. Eight did not contain the name and phone number of the child's physician. 3. Five did not contain information for the family’s preferred hospital. The staff person in charge identified all of the children as being enrolled since 8/16/21, and stated that most of the parents filled out enrollment information online through the school. The staff person in charge went to the school office during the inspection in an attempt to obtain some of the missing information; however, no one was available to assist her at the time. General: Based on observation and interview, this regulatory requirement was not met. A small area between Classroom #202 and the facility office contained a sink that was coated with layers of dirt and grime. A toilet brush was positioned on the floor beside the sink and a toilet plunger was observed positioned on the rim of the sink. The staff person in charge stated during interview that this space is used by the cleaning personnel at the school. Gates that had been positioned in the doorway to the classroom and the doorway to the office were standing open throughout the majority of the visit; therefore, the space was accessible to the children. Furthermore, the surveyor observed a staff person change a child's clothes in this space and then use the sink for hand washing. Review of staff files revealed one (1) employee with a hire date of 9/23/2022, did not have documentation of the minimum educational requirements. General: Based on observation and interview, the child care center failed to maintain state regulations in regards to inaccessible items. 1) Observation of the Three's and Four's room revealed a container of Lysol cleaning wipes on an accessible window sill, located behind the teacher's desk. 2) Observation of the Four's and Five's room revealed a bottle of hand sanitizer, a spray bottle labeled "soap", and a container of hand sanitizing wipes located on a small bookshelf to the immediate left of the entrance door. 3) Further observation of the Four's and Five's room revealed a set of keys and a backpack on a small green table in the front of the classroom. An interview with the center director revealed the keys and backpack belonged to staff.
General: Based on interview and review of documentation, the child care center failed to maintain children’s records in accordance with regulations. Review of children’s files revealed the immunization certificates presented for review at the time of survey for four (4) children (3/23/2018, 5/6/2018, 10/30/2017 and 9/20/2017) are no longer current. Interview with the director revealed the center needs to obtain a current immunization certificate for these children. One staff person (hire date: 8/4/21) had a completed fingerprint-based background check in KARES with eligible for hire status; however, did not have evidence or results of an out of state background check from Ohio. The staff person had a copy of an Ohio driver’s license in her file; therefore, she would need a child abuse and neglect check and a sexual abuse registry check from this state. A review of Division of Regulated Child Care files found that this regulatory violation was previously issued as a result of the initial inspection conducted on 8/4/21. The staff person in charge stated that she submitted the necessary out of state background checks for this staff person; however, had not yet received the results and was not able to provide any documentation to the surveyor during the visit. Two steam radiators in Classroom #200 had been covered across the front with a foam board; however, the top of each radiator was not protected and were accessible to the children. These radiators were turned on and hot to the touch during the inspection. A toy was observed positioned on top of one of the radiators.

Several inches of the seat area had broken off and was missing. The door to the stall was locked during the inspection. The staff person in charge acknowledged the finding and stated that the toilet is not currently used by the children.
