General: Based on observation and interview, the following was found: Young Two's Classroom 1. A windowsill had cracked lengthwise resulting in a jagged edge and chipped paint. PreK 1 Classroom 1. The floor of the restroom used by children in care had areas of rust-colored staining. The staff in charge stated that the floors of the center were in the process of being stripped and rewaxed. PreK 2 Classroom 1. There were strips of tape residue on the floor around the edges of a blue rug that had begun to collect debris. The staff in charge stated that the floors of the center were in the process of being stripped and rewaxed. Two's and Three's Classroom 1. The wall to the right of the restroom used by children in care had several areas of damage to a foam wall covering. 2. The back wall of the classroom near the closet had a hole in the drywall that had not been patched. 3. The wall near the alphabet rug had areas of damage and chipped paint. The staff in charge stated that renovations to the walls in this classroom were in process.

PreK 1 Classroom 1. A cabinet in the dramatic play center had a hole in the top that extended through the width of the wood surface, resulting in a jagged edge. 2. General: Based on interviews and review of video footage, on the date of 02/03/26, two (2) children are seen on top of a bean bag in the corner of the Two's and Three's Room. One ( 1) child is on top of the other, using a toy stethoscope to examine his private area. The staff member in the room quickly intervenes and re-directs the children; however, when she walks away the two (2) children resume exactly what was happening before the intervention. At this time, the staff walks back over to the children, but does not intervene, and watches them for approximately twenty-two (22) seconds before redirecting them. As she walks away, the two (2) children resume what they were doing again, and the video abruptly ends.
