General: Based on observation and interview, the child care center failed to maintain adequate supervision of children at naptime in the Bardstown 2 classroom. Observation revealed one (1) staff at a teacher’s desk and another staff at a computer with his/her back to the children who rested. The surveyor stood at both positions of the staff and where it was revealed children rested out of the scope of vision of the staff who cared for them. One child was located in the upper far right corner of the classroom behind a shelf and another child was located between a shelf and table with chairs, in the middle area of the classroom. Interview with staff revealed the children ranged in age from three (3) to four (4) years old. General: Based on observation and review of documentation, the refrigerator in the child care center cafeteria contained food that was served to children in care. Review of the temperature measuring device for the refrigerator revealed a temperature of fifty (50) degrees Fahrenheit, upon initial inspection. The surveyor checked the refrigerator again, one and one-half (1 ½) hours later and the temperature was forty-five (45) degrees Fahrenheit. The temperature in the refrigerator exceeded the regulatory requirement of forty (40) degrees by at least five (5) to ten (10) degrees. Interviews and review of documentation consistently revealed a three-year-old child was found alone, in a classroom, out of the scope of vision and range of voice of any supervising staff members, for approximately five (5) minutes; the aforementioned child's classmates and two (2) supervising staff members were on the playground and did not realized the child was left in the classroom until brought to their attention by administration. Review of two (2) employee files revealed the center failed to provide documentation at time of survey to verify completed child abuse or neglect checks using the central registry in accordance with 922 KAR 1:470, as well as, criminal records checks required by KRS 199.896(19 for both employees hired 7/31/15 and 8/10/15. Based on documentation review it was revealed that the center could not verify that the criminal records check (CRC) had been completed for the volunteer with a start date of 08/19/2015. The volunteer was observed working with two (2) staff members present in the classroom. Continued review of documentation revealed the staff with the hire date of 08/11/2015 had resided in the state of Tennessee within the last five (5) years and the center had not completed an out of state background check.
