The provider on 2/27/25 had two children age 16 months and 13 months in cribs sleeping with a black mesh breathable cover on top as well as blankets on the outside that blocked the site of the children for the rise and fall of the chest. On 3/6 the children were observed in the cribs and the provider is no longer using the mesh covers or the blankets on the outside.
