On 5/1/2025 and during today’s visit unqualified caregiver A.G. was on site prior to a national criminal history check being completed.
On 5/1/2025 and during today’s visit unqualified caregiver A. G. was on site and did not have a physical or TB test on file.
On 5/1/2025 a caregiver hit a one-year-old child on the butt and flicked the child in the face. A caregiver swung an object near a one-year-old child’s face while the child was sitting in a car seat. A caregiver threw a blanket over a one-year-old child’s head. A caregiver picked a child up by the wrist and placed the child on a chair. The caregivers that were present used profanity in front of the children and a caregiver cursed at a child. A caregiver yelled at some of the children and told a child to shut his mouth.
On 5/1/2025 and during today’s visit unqualified caregiver A.G. was on site and has not completed child abuse and neglect training.
During today’s visit one child was inside of the home in the bathroom while the caregiver and the children were outside.
