At time of visit, janitor closet door frame had a magnet attached which prohibits door closing. Door was unlocked. Closet contained multiple cleaners and disinfectants.

Restroom in preK classroom had open container without lid storing clothing. Broom set in restroom.
Self report on 3/6/26, by director reported a 4 year old child left the playground and returned back inside the building into classroom. Child was reported to be in the classroom left unattended for 4 to 5 minutes without qualified caregiver. Caregiver also reported that 4 other children had ran through the PreK gate, Toddler playground gate to the front of the building playground. The classroom did not have the proper number of qualified caregivers directly caring for the number of children in the classroom. They had 0 qualified caregiver with 1 child and the required ratio for that age group 1:13.
Complaint on 3/5/26 reported caregiver grabbing 4 year old child by the arm and leg restricting movement. Caregiver reported that she restricted movement by grabbing child's arm and leg to lift child up off the cot to remove child from the classroom.

At time of visit, consultant observed caregiver grab 3 year old child by arm and drag across the classroom putting him in chair with force.
At time of visit, the cots in the PreK classroom were not a minimum of 2 feet apart on all sides.
At time of visit, consultant observed caregivers in the PreK classroom using harsh tone, yelling and telling children "You're done and I'll be writing you up. You know better. You need to just stop".
The provider does not have documentation of a criminal history of 4 caregivers present at the time of inspection.

Twos restroom contained broom set, and laundry basket with towels. Threes restroom contained toilet brush. School-age restroom contained plunger and winter coat.
Items labeled "Keep out of reach of children" were accessible to children. Unlocked closet contained Dawn Professional soap gallon, 7 gallons of floor cleaner, and 4 gallons of multi-surface cleaner. Twos classroom- unlocked cabinet contained caregiver's purse. School-age classroom- open cabinet contained caregiver's purse.
