Only Qualified caregivers count for supervision and ratio. repeat citation- During visit consultant obeserved the Toddler class out of ratio with 7 toddlers with 1 caregiver. Additional caregiver left the classroom. Ratio is 5:1

repeat citation- on school age playground orange internet cable runs across the width of the playground from building to outside fencing causing a tripping hazard. At time of viist, broken pipes and wood pieces on the ground under utility boxes on school age playground. Bags of top soil accessible to children.
At time of visit, kitchen door was open and accessible to 4 year old children in adjoining classroom.
Beehive present on corner of building by gutters on the toddler playground. Open piping sticking out of brick section of building that are open outdoor on playgrounds.
At time of visit, childrens files missing required documents. Consultant will return to review files in 30 days.
At time of visit, Laundry room ceiling has water damage around dryer vent. Water damage appears to be present by electrical boxes.
Screens on windows have tears and holes. Doors leading out to playground in school age and preschool classrooms do not close, can be pushed open due to not latching shut.
At time of visit, Toilet paper and paper towels missing in preschool restroom.
Items labeled "Keep out of reach of children" were accessible to children. PreK classroom- Open cabinet contained Ibuprofen, Handsanitizer foam, multipurpose cleaner, clorox wipes, restroom cleaner. Preschool classroom- Teacher's purse on desk, and shaving cream on shelf. Open kitchen- contained bleach gallons, restroom cleaner on the floor. Raid spray on the counter.
At time of visit, staff files missing required documents in files. Consultant will return in 30 days to review.

During the visit consultant observed the toddler classroom out of ratio on two seperate occassions. Both times leaving the classroom with a 7:1 ratio. Ratio for toddlers is 5:1
