In the toddler room, and the 3s classroom, the changing pad is unsanitizable due to cracking of the plastic barrier.
At time of inspection, playground was not properly draining, creating standing water which is unsafe for children.

A provider shall provide for a safe environment by ensuring that no conditions exist in or on the grounds of the facility where a provider operates a child care program that would endanger the health, safety, or welfare of the children, including ensuring that the following items are placed in areas that are inaccessible to children in the providers care: Medications
In infant classroom, teachers purse was found on the kids toy shelf within reach of children which is not safe. In toddler classroom, teachers purse, hand sanitizer, and clorox wipes were within reach of children, which is unsafe. In the 4/5s class, lysol spray and clorox wipes were within reach of children which is unsafe. In the 2nd 3s classroom, lysol spray and hand sanitizer were within reach of children which is not safe. These items are labelled "keep out of reach of children".
A provider that cares for more than sixteen (16) children at a facility where the provider operates a child care program shall maintain a ratio of children to caregivers in the same proportions as specified in the child to staff ratio requirements.
At time inspection, infant fridge reading 55 degrees.
In the hallway bathroom, toilet paper was open on the back of the toilet and dispenser was empty at time of inspection, this is not sanitary.

Infant/Toddler room: Sink caulk is cracked, and the sink is able to be pushed off from the wall which is not sanitary or safe. Send documentation to consultant. Toddler/2 room: the blue highchairs in the table do not have straps which is unsafe. PreK room bathroom There is a water-stained ceiling tile and in the Preschool room there is a water stained ceiling tile which is unsanitary. In the school-age room one outlet plastic cover is broken exposing the wires which is unsafe. Send documentation to consultant.
Director is missing Safe Sleep trainings 1 and 2 in their file. (A.T.) Send documentation to consultant.
Each childcare provider shall have documentation of a Drug test and result does not show presence of illegal controlled substance(s).(Standard 5 or 8 panel urine test)

COMPLAINT: On 2/14/24, caregiver forcefully covered child’s face with blanket.

In the dining room under the sink there is Fabulaso and Lysol toilet bowl cleaner and in the Blue cabinet there is Window cleaner and soap labeled “keep out of reach of children” accessible to children which is unsafe. In the Infant room a roll of paper towels sitting on a shelf and not in a dispenser which is unsanitary. Outlets are uncovered in the 1’s-2’s room, School-age room and dinning area which is unsafe. In the dinning area there is a hole in the drywall from the door handle which is unsanitary.

The Infant room changing table door on the right is broken and falls off which is unsafe.
In the Infant room the seal around the sink is broken which is unsafe. Roll of paper towels uncovered sitting on a box which unsanitary. Infant room, PreK room, 2’s, and 1’s rooms have peeling paint and dents in the wall which is unsanitary. In the 1’s room the black rubber carpet seal is loose and a tripping hazard. In the 2’s room there are 2 ceiling tiles with water stains and the air vent is dirty which is unsanitary. The 4’s room floor tiles are broken which is unsanitary and the gold metal threshold is broken which is a tripping hazard. In the 3’s room there is Shaving cream on the toy shelf accessible to children which is unsafe. In the 3’s room the dark green wall has a dent exposing drywall which is unsanitary.
