On the playground there were several pieces of broken plastic on the playground. The plastic playhouse that is broken. The mulch is then exposing the dirt, and weed growing through. Pieces of popped balloons on the playground.

In the Infant room bathroom, cleaning supplies, Clorox Bleach Spray, Windex spray, Scrub Free cleaner stored under the bathroom sink.

Safe Sleep Practice was not followed when Consultant observed a 7 month old male asleep in crib in the infant classroom with a blanket, and Consultant observed a 6 month old female asleep in crib in the infant classroom with a blanket.
Lighting in the infant room was too dark. Consultant could not see the children in the cribs or see the rise and fall of their chest.
Rest area lighting did not maintain an average of 5 foot-candles of light in all areas. In the 3-5 room, Consultant could not see staff member sitting in corner of the room. In the infant-3 room, consultant had to turn on phone flashlight to see child sleeping in crib.
Consultant observed one staff member sleepoing on the floor. Due to staff member laying on the floor sleeping, supervision was not being maintained.
